Summary
On July 28, 2023, The Office of Defects Investigation (ODI) opened a Preliminary Evaluation (PE23-014) after receiving 12 Vehicle Owner Questionnaires (VOQs) reporting loss of steering control in model years (MY) 2023 Tesla Model 3 and Y vehicles. During PE23-014, ODI interviewed complainants, sent an Information Request (IR) letter to Tesla
, and convened technical meetings with Tesla
that included a joint inspection of a crashed vehicle. The investigation identified 2,388 total complaints regarding the alleged defect, including 124 complaints through VOQs and 2,264 complaints through manufacturer data. ODI is associating 1 crash to this investigation (11544655), where a driver was unable to complete a right hand turn in the intersection and hit a vehicle. This vehicle was subject to a joint inspection by ODI and Tesla
. The steering rack was recovered by ODI with the consumer's permission. ODI's interviews of allegations supplied to the agency, in the form of VOQs, identified patterns of the incoming complaint traffic. Complaints allege an inability to turn the steering wheel, while other complaints allege an increase in required effort to turn the steering wheel. These conditions were reported to occur during the drive cycle as well as at start-up. Approximately one-third of complaints with a reported speed occurred at vehicle start-up or speeds less than 5 mph. Interviews with failures reported at speeds over 5 mph, half reported an inability to turn the steering wheel. A majority of complaints with a reported speed reported between 5 mph and 35 mph with approximately 1 in 10 occurring on open roads at speeds 35 mph and over. The highest reported speed regarding an alleged inability to turn the steering wheel is 75 mph. A majority of allegations reported seeing a warning message, "Steering assist reduced", either before, during or after the loss of steering control. A portion of drivers described their steering begin to feel "notchy" or "clicky" either prior to or just after the incident. There have been multiple allegations of drivers blocking intersections and/or roadways. ODI is aware of over 50 vehicles which were allegedly towed as a result of the condition from a variety of areas including, driveways, parking lots, side of road and intersections. Multiple complaints state they were able to temporarily remedy the condition by power cycling the vehicle, but the issue reoccurred until the steering rack was replaced. Tesla
supplied four steering rack part numbers on the subject vehicles. The investigation identified part numbers, 1044832-00-A and 1188832-00-A accounted for 2,187 of the complaints and 4 did not have original part number in the information request. In a letter dated July 2, 2023, Aldelberto A. Cordova (the petitioner) submitted a petition requesting that NHTSA initiate an investigation of an alleged defect within the "ISG 48-volt on-board electrical system," which resulted in warning light illumination as well as an inability to start the vehicle in a 2023 Mercedes-Benz GLC300. ODI reviewed the petition, complaint data in NHTSA's databases and information gathered from an Information Request (IR) letter sent to Mercedes-Benz
. Examination of this information indicates that the subject component listed in the petition is not capable of causing the a loss of motive power. The alleged defect cited in the petition was caused by a software deviation in the Central Powertrain Controller (CPC), which is a separate system and independent of the 48-volt system. After thoroughly assessing the material submitted by the petitioner, information in NHTSA's possession, information submitted by Mercedes-Benz
, NHTSA does not find that a formal investigation is warranted. Accordingly, the Agency is denying the petition. As with all potential motor vehicle safety risks, NHTSA will continue to review any new information or incidents as they are submitted to the Agency. Summary Description: Failed Door Check Arm Attachment Point
Summary
On July 7, 2023, the Office of Defects Investigation (ODI) opened PE23-013 to investigate instances of check arm bracket failure in the front doors in model year (MY) 2020-2021 Ford Escape vehicles. At the time of opening, ODI received 118 Vehicle Owner Questionnaires (VOQs) alleging this failure. To date, ODI has received 593 VOQs alleging this failure, with a variety of reported failure conditions. The most common reported failure condition was reduced door functionality (e.g. door was more difficult to open or close) and the least common was inadvertent door opening. Other reported failure conditions include inability to latch and inability to open. Three VOQs reported injuries, one of which requiring medical attention. ODI conducted a consumer survey which yielded an additional 44 injuries (although the VOQs did not indicate any injury), none of which requiring medical attention. All these injuries were bumps/bruises from the door closing on occupants' arms and legs while entering and exiting the vehicle. The function of the check arm system is to provide hold-open assist at specific angles and to assist with door closing energy. It does not provide vertical support of the door and is independent of the latching mechanism. This failure is progressive and detectable by vehicle occupants. Consumers commonly report hearing a popping or snapping noise when opening and closing the door. This noise is caused by some of the welds between the bracket and door structure failing and detaching. There may also be visible damage present to the welds in the door structure. Continued use of the door may result in all welds separating and the bracket fully detaching from the door structure, which could lead to increased resistance in closing and opening the door, restrained window operation, and the door freely rotating when open. ODI inspected 3 consumer vehicles in the field, and 2 test vehicles at Ford's testing facility. The test vehicles were retrofitted with the failed component. ODI observed an increased door closing resistance, however, was unable to reproduce an inadvertent open while parked or driving, or a failure to latch or open condition. Testing also verified this failure did not affect the door ajar warning system. This system provides an audible and visual warning in the event a door did not achieve primary latch position. Ford
has provided details on a one-time-repair customer satisfaction program (23B56), stating that they will provide customers with a letter, currently targeted for March 2024, to bring their vehicle to a Ford
dealership for a proactive repair. Dealers will inspect the door check arm welds for damage and will either provide a rivet plate to reinforce the existing door check arm or will provide a replacement door, depending on the level of damage present. Customers will be eligible for reimbursement if they previously paid to repair this issue. In view of the high detectability of failure, low potential hazard to drivers, and Ford's customer satisfaction program to provide a proactive repair, this Preliminary Evaluation (PE) is closed. The closing of this investigation does not constitute a finding by NHTSA that a safety-related defect does not exist. The Agency reserves the right to take additional action if warranted by future circumstances. On July 3, 2023, the Office of Defect Investigation (ODI) opened PE23012 to investigate allegations of loss of motor power on model year (MY) 2022 Kia EV6 vehicles. Some consumers reported loss of motor power immediately after warning lamp illumination or reduction of motive power while the vehicle was in motion. This investigation found that the Integrated Charing Unit (ICCU) may become damaged over time from transient high voltage and thermal cycling. A damaged ICCU may not be able to charge the 12-volt battery which can discharge gradually while driving with a reduction of motive power. The vehicle could eventually experience a complete loss of motive power thereby increasing the risk of a crash. On July 13, 2023, Kia
conducted a service campaign to inspect the ICCU and install an improved over-current detection software logic in certain 2022-2023 EV6 vehicles. On March 15, 2024, Kia
filed recall 24V-200 for 48,232 MY 2022-2024 Kia
EV6 vehicles. The recall remedy provides an inspection of the ICCU, and if necessary, a replacement of the ICCU and the associated fuse. The ICCU software reduces the risk of overcurrent and implements voltage peak reduction at the end of EV battery charging. The recall also includes a software update that revises the electric water pump operational threshold to reduce thermal loading during charging and driving.
