Summary
The subject vehicles have electro-hydraulic power assist steering (EHPAS).The complaint counts shown above alleged a loss of power steering assist in the subject vehicles.Usually, but not always, the loss is accompanied by the simultaneous lighting of an indicator light on the dash.The complaints include 14 alleged crashes involving 6 injuries.On Mazda3 and Mazda5 vehicles manufactured from April 2, 2007 through November 30, 2008, rust may have formed inside the EPAS high-pressure pipe during manufacture due to improper processing.If the rust is present it may damage the power steering pump bearings leading to an increased load on the electric motor; this results in an elevated operating temperature for the EPAS system.To prevent excessive overheating of the system, which could potentially cause permanent damage, afailsafe mode which de-powers the electric pump is implemented.This causes a loss of power assist making it more difficult to steer the vehicle, especially at lower vehicle speeds.Once the failsafe is implemented the EPAS system will remain disabled until the ignition is turned off.However EPAS will be re-enabled if the operating temperature is reduced (i.e., within an acceptable range) on a subsequent drive cycle.On August 8, 2010 Mazda submitted a Part 573 defect information report initiating Safety Recall 10V-374 to remedy the subject vehicles.The report, which contains additional details on the remedy repair, is available at http://www-odi.nhtsa.dot.gov/recalls/.ODI notes that the affected population shown in that report includes approximately 8,000 vehicles Mazda repaired prior to initiating the recall.

Summary
On May 11, 2010, Skyline Corporation notified the NHTSA that it had decided certain model year 2004-2007 Skyline Park Model trailers contain a safety related defect.Skyline stated that certain trailers were originally equipped with Dimplex electric fireplaces containing a plug-in remote control receiver that can overheat, possibly causing a fire.NHTSA assigned recall number 10V-201 to this matter.In early May 2010, Thor Industries contacted the Recall Management Division (RMD) regarding the use of these Dimplex fireplaces in several of its recreational vehicles.Upon initial contact with Dimplex, it was learned that these fireplaces were sold through a distributor that may sell them to many other recreational vehicle manufacturers to be installed as original equipment.Dimplexissued a recall with the U.S. Consumer Product Safety Commission (#10-167) for 700,000 units.In May 2010 RMD informed Dimplex of its responsibilities under 49 CFR 573 as to its obligations to conduct a safety recall for the original equipment used in motor vehicles by more than one vehicle manufacturer.Dimplex submitted its Part 573 Report (10E-027) on June 6, 2010, identifying a total 25,000 defective units sold to its distributor, WESCO International, Inc. (WESCO).On July 6, 2010, RMD mailed WESCO an Information Request (IR) letter requesting a full list of customers that were sold these recalled fireplaces.WESCO responded on July 23, 2010, with a listing of 162 companies that were further sold this equipment.After parsing out closed companies from the list and those known to RMD not to further distribute motor vehicle equipment or manufacture vehicles, RMD contacted a total of 40 companies with IR letters mailed on August 31, 2010.On November 1, 2010, RMD mailed a second round of IR letters to 14 companies that had not yet responded to the first letter.By the end of December 2010, another 2 IR letters were mailed to Wray Brothers and Rexhall Industries, as they had still not responded to RMD's letters.As of the closing of this EQ, despite multiple phone calls, e-mail exchanges, and IR letters, these two companies have still not filed an official response with RMD regarding this matter.RMD will refer this matter to the Office of the Chief Counsel (NCC) for further enforcement activity as appropriate.Of the remaining 38 companies RMD contacted, their responses break down as follows:- 23 vehicle manufacturers filed a Part 573 Report with NHTSA and contacted their affected customers.- 6 letters were returned as those companies are no longer operating.- 4 companies only used the units for samples/prototypes and did not retail them to the public.- 3 companies either did not receive any affected units or only sold their units outside the U.S.market.- 1 company, R-Vision, responded that their company filed bankruptcy and they no longer possessedcustomer records for those vehicles affected by the Dimplex equipment.- 1 company, Custom Campers, is owned by NuWa Industries and their affected units were included in NuWa's Part 573 report to NHTSA. Unsecured mats may interfere with accelerator pedal return to idle. A heel blocker in the floor pan provides a platform that may lift an unsecured mat into contact with the pedal. Ford introduced new pedals as a running change early in model year (MY) 2010 vehicles. Analysis of complaints received by ODI and Ford show elevated rates of pedal entrapment incidents in MY 2008 through early 2010 production vehicles. Incidents typically occur following hard pedal applications to pass slower traffic or when merging into faster traffic. Drivers allege continued high engine power after releasing the accelerator pedal and difficulty braking, including reports that the incident was controlled by shifting to neutral or turning the engine off. Drivers and service technicians reference observing evidence of mat interference or note unsecured Ford or aftermarket all weather floor mats in post-incident inspections.Based on information gathered during the Preliminary Evaluation the population being investigated has been narrowed to model years 2008 to 2010.This investigation has been upgraded to an engineering analysis on MY 2008 through early-2010 vehicles (produced through September 2009) to further assess the scope, frequency and safety consequences of the alleged defect.The VOQs associated with the opening of this Engineering Analysis are: 10479693, 10415911, 10379562, 10372674, 10345830, 10342572, 10339330, 10335336, 10334211, 10332233, 10331160, 10329659, 10327597, 10320011, 10232027, 10214131

This investigation was opened after ODI became aware of four engine compartment fires allegedly related to fuel leakage from the engine's fuel injector system.ODI conducted field examinations and communicated the identified concerns to Blue Bird.ODI also had discussions with Caterpillar, the engine manufacturer.On June 28th, 2010, Blue Bird filed two safety defect reports with NHTSA to address fuel lines that may crack at the fastening point to the common fuel rail. Safety recall campaign 10V-289 involves 3,686 school buses and safety recall campaign 10V-290 involves 103 non-school bus vehicles. The subject vehicles were equipped with the Caterpillar C-7-07 engine and it was determined that clamping of the #5 fuel line to the #6 fuel line and assembly stresses caused the #6 fuel line to fail. The remedy consist of replacing the #5 and #6 fuel lines and adding a new clamp to # 5 fuel line. Simultaneously with Blue Bird's decision to conduct the above recalls, Caterpillar Inc. filed a safety recall (10E-025) with NHTSA to perform the same remedy on 5,920 C7 series engines.Blue Bird engines are included in the Caterpillar recall.These recalls resolve the issues raised by this investigation. Therefore, this investigation is closed.

Summary Description: Interior Door Handle Laceration Risk
Summary
The investigated concern involved chrome plating on interior door handles that could peel or separate from the plastic base material.This de-lamination could result in a sharp edge and pose a risk of laceration.In some cases, consumers reported minor hand/skin lacerations.Of these reported injuries, the vast majority of injuries were to adults.Ninety percent of claims associated with interior door handle de-lamination were related to the front doors.In addition to the above Tahoe/Yukon vehicles, other GMT 900 series C/K vehicles (Avalanche, Escalade, Sierra, Silverado, and Suburban) have a trim level that utilizes the chrome plated interior door handles and were included in the analysis.The resultant population group had a total of 746 complaint reports with 309 injury incidents and 43,619 unique VIN regular warranty claims over a combined 451,745 vehicle population.The door handle supplier was changed in September 2006 with new tooling and process in place by October 2006.The base population prior to the September 2006 vendor change accounted for the bulk (or 89%) of all complaints and comparable injury reports and warranty claims.Due to the nature of the failure (thin chrome plating peeling off the door handle), only minor hand/skin laceration injuries similar to a paper cut were recorded.All but a few did not require professional medical treatment such as surgical or liquid sutures (i.e. stitches).Less than 10% of injury reports had specifically involved a 3-10 year old child.This was due to the fact that 90% of the claims were associated with door handles located in the front seating area (where the window glass panels usually have less solar tinting than rear window glass panels) where children are not normally seated. In July 2010, GM issued a Special Coverage Policy (SCP 09239) on the base vehicles to extend the warranty coverage to 10 years/100k miles.Recent data indicate that the base population complaint and injury reporting have subsided since the implementation of the SCP campaign.In June 2011, GM added the remaining interim population (Sep-Oct 2006) to the SCP.A safety-related defect has not been identified at this time and further use of agency resources does not appear to be warranted.Accordingly, this investigation is closed.The closing of this investigation does not constitute a finding by NHTSA that a safety-related defect does not exist.The agency will monitor this issue and reserves the right to take further action if warranted by the circumstances.
