ODI found the rate of warranty claims related to fuel spit back on the subject vehicles is lower than other vehicles previously recalled to correct defects related to fuel spit back.Although the rate of complaints reported to the agency on the subject vehicles is higher than or similar to the rates experienced in previous investigations where safety recalls were conducted, ODI suspects that the NHTSA safety complaints submitted by subject vehicles owners may have been influenced by internet related publicity both before and during this investigation.A safety-related defect trend has not been identified at this time and further use of agency resources does not appear to be warranted.Accordingly, this investigation is closed.The closing of this investigation does not constitute a finding by NHTSA that no safety-related defect exists.The agency will monitor the issue and reserves the right to take future action if warranted by the circumstances.See attached report for additional information.In addition, ChryslereBay is initiating a lifetime warranty program, at no cost to the consumer, to address the fuel spit back problem in approximately 135,000 model year 2007-2008 JeepeBay Wrangler vehicles built from March 1, 2007 to March 31, 2008.ChryslereBay will notify all affected vehicle owners of this action.Consumers should refer to the warranty and service bulletins and sample owner notification letter available in the investigative file.The vehicle build date can be found on the certification label located on the driver door or door jamb.

Summary

The subject vehicles suffered from a design issue whereby water could run down onto the cowling under the hood and located in the area of the windshield wipers and then drip onto a computer module, the Powertrain Control Module (PCM), which could be manifested in a variety of symptoms depending on which part of the circuit board became wet or which of the connector pins shorted out.In addition to causing an engine to stall, which may or may not then restart, the water intrusion could also cause the transmission to shift poorly, the vehicle to have erratic power, a malfunction indicator light to come on, or result in an inability to start the vehicle, an inability to shift the vehicle into either forward or reverse. Also the problem could cause a higher than normal idle RPM in Park, or for the radio to malfunction, or the interior lights to stay on.During this investigation ODI was concerned with the effect of the engine stalling or a drastic and sudden loss of power either of which may create a safety risk.In reviewing the actual number of these types of incidents, collectively referred to here as engine stalls, the number was found to be below what would be considered a defect trend.For example, the number of engine stalls identified in FordeBay's warranty claims for the model year (MY) 2004 subject vehicles, the MY with the most failures, was 200 out of population of 125,865, for a failure rate of 0.16%.The one crash recorded by a complainant to NHTSA, VOQ 10311522, alleged that the engine stalled, due to water intrusion into the PCM, while the road was turning causing her to go off the road and according to the police report the vehicle then hit an embankment and a tree stump. FordeBay reported in their response that there were three alleged crashes but none of these were validated as being caused by a problem with water intrusion into the PCM causing a stall.Accordingly this investigation is closed.The closing of this investigation does not constitute a finding by NHTSA that a safety-related defect does not exist.The agency will continue to monitor complaints and other information relating to the alleged defect in the subject vehicles and take further action in the future if warranted.

Summary Description: WebastoeBay Sunroof Separation


Summary

On August 10, 2010, WebastoeBay notified the NHTSA that it had decided certain Hollandia 600/700 & Hollandia TVS 900 aftermarket sunroofs contained a safety related defect. WebastoeBay stated that these sunroof models, manufactured between 2002 and 2010, may experience an adhesive bond failure where the sunroof glass attaches to its frame. This failure increases the risk of the glass detaching from the vehicle and causing injury to other motorists or pedestrians.This recall was assigned number 10E-038. WebastoeBay and its counsel met with RMD on August 18, 2010 to discuss the recall's next steps. RMD learned that the majority of these products are installed by a third party, on new motor vehicles that have not yet been retailed, often at the request of a new car dealership. These installers are required to notify the NHTSA of any new vehicles they altered by way of a Part 573 Defect Report. On September 2, 2010, RMD issued an information request (IR) letter to WebastoeBay requesting a list of companies who purchased the defective sunroof models.WebastoeBay replied on September 9, 2010, submitting a purchaser list to NHTSA's Office of the Chief Counsel (NCC) for confidentiality consideration.NCC granted this confidentiality request as the release of WebastoeBay's purchaser list could cause substantial competitive harm.Due to the large number of companies that purchased these sunroofs, RMD worked with WebastoeBay to ensure notifications to WebastoeBay's authorized installers (AIs) included instructions on how to file a Defect Report with NHTSA should one be required.For those AIs that altered new vehicles (vehicles not yet delivered to the first retail purchaser), the AI notification letter offered guidance on how to submit a Defect Report to NHTSA and instructions that a third party, Impartial Services Group, LLC (ISG), would be contacting them to obtain VIN information for vehicles they altered as new.ISG obtained VIN information from each AI and returned to them a computerized list of vehicle year, make, and model as this information is necessary for a Defect Report submitted to NHTSA.ISG then utilized the acquired VIN information to notify the current registered owners about the safety recall. On July 5, 2011, RMD mailed IR letters to 87 AIs that had previously communicated to WebastoeBay that they had maintained vehicle VIN records for their past alterations, but had not yet sent those records to ISG.Forty-five (45) of these companies did not respond to RMD?s IR letter, however, some did take action by submitting their records to ISG.RMD chose to mail a second IR letter on April 19, 2012, to 8 AIs who were identified as having the largest quantities of sunroof installations that needed to be recalled.Four (4) companies responded by filing Defect Reports with NHTSA and sending their affected VINs to ISG for consumer notification.The information for the 4 nonresponsive companies: Automotive Concepts, Classic Auto Interiors & Accessories, Dundalk Glass Center, and Kenny?s Auto Stylist will be forwarded to NCC for possible enforcement action at their discretion. With an ISG generated vehicle list and a Defect Report, RMD received safety recalls from 111 AIs recalling over 74,000 vehicles.It is very likely that more AIs contacted ISG and WebastoeBay to submit their records of affected vehicles, but failed to notify the NHTSA of their actions.WebastoeBay has mailed over 231,000 owner notification letters to vehicle owners that have recalled sunroof assemblies.This investigation is concluded as we believe we have realized the safety benefits that can be reasonably achieved through an Equipment Query of this scope.This EQ is cl

On August 26, 2010, ToyotaeBay submitted a Defect Information Report to the National Highway Traffic Safety Administration (NHTSA) regarding problems with Engine Control Modules that could result in engine stall while driving in certain model year (MY) 2005 through 2008 ToyotaeBay Corolla, Corolla Matrix and PontiaceBay Vibe vehicles equipped with the 1ZZ-FE engine and two-wheel drive and manufactured between April 19, 2004 and January 2, 2008.ToyotaeBay's report indicated that cracks may develop at certain solder point or on varistors on the ECMs.According to ToyotaeBay, such cracks generally resulted in engine warning lamp illumination, harsh shifting or engine no-start.In some cases, if the cracking occurs on particular solder points or varistors, the engine could stop while the vehicle is being driven.ToyotaeBay will notify owners of subject vehicles that they should return their vehicles to a ToyotaeBay or PontiaceBay dealer.Dealers will be instructed to inspect the production number of the ECM for each vehicle and replace the ECM if necessary.On November 30, 2009, the Office of Defects Investigation (ODI) opened a Preliminary Evaluation (PE09-054) to investigate alleged engine stall while driving in model year (MY) 2006 ToyotaeBay Corolla and Corolla Matrix vehicles.In its response to ODI's information request, submitted on March 2, 2010, ToyotaeBay indicated that it had identified two possible conditions with ECMs used in MY 2005 through 2007 ToyotaeBay Corolla and Matrix vehicles:1) cracks forming between the IC and the board caused by an improperly cured conformal coating applied to the circuit boards after the soldering process is completed; and 2) cracks occurring in a variable resistor (varistor) due to overheating.During the manufacturing process a glass coating is created on the surface of the varistor.Between March and August 2010, ToyotaeBay began collecting ECMs from the field to further investigate failure modes that may result in engine stall.ToyotaeBay also conducted durability testing on ECMs collected from in-use vehicles to assess whether the problems may be a continuing trend. In addition, they conducted durability testing in order to assess whether this problem was a continuing trend.ToyotaeBay found that 4 of 32 collected ECMs malfunctioned after thermal shock testing.ToyotaeBay also confirmed that cracking could develop at solder points on the ECM circuit board that could lead to engine stall while driving and that this trend was likely to continue.The manufacturer failure data in this resume is limited to MY 2005 through 2007 ToyotaeBay Corolla and Corolla Matrix vehicles.Based on ToyotaeBay's recall, this Engineering analysis has been closed.

The Office of Defects Investigation (ODI) is aware of a single complaint on the subject vehicles that alleges the hand wheel became mechanically de-coupled from the road wheels when the intermediate steering shaft universal joint separated from the upper steering column.It appears that the intermediate steering shaft in the complaint vehicle was disassembled and then improperly re-assembled with the universal joint not fully engaged with the upper steering column's splined shaft and the pinch bolt was not secured in the retaining groove when fastened.The component supplier has added additional fool-proofing measures to the assembly process to further reduce the likelihood of improper assembly.KiaeBay inspected a total of 3,695 subject vehicles and steering column assemblies at the component supplier, the vehicle assembly plant in Korea and at US ports in the weeks immediately following the opening of this pursuit and found no evidence of improper assembly.KiaeBay also conducted a service action (SA057) to inspect another 169 subject vehicles (out of 336) manufactured on the same day as the complaint vehicle and a total of 594 subject vehicles (out of 1,747) manufactured during the same week as the complaint vehicle and found no issues to report.An additional 342 subject vehicles outside the scope of the service action were also inspected and again KiaeBay found no evidence of improper assembly.A safety-related defect trend has not been identified at this time and further use of agency resources does not appear to be warranted.Accordingly, this investigation is closed.The closing of this investigation does not constitute a finding by NHTSA that a safety-related defect does not exist.The agency will take further action if warranted by the circumstances.
