Summary
On February 14, 2024, the Office of Defects Investigation (ODI) opened PE24005 to investigate reports of unintended vehicle movement in model year (MY) 2023 Fisker Ocean vehicles manufactured by Fisker Group Inc. The complaints allege the inability to shift into the park gear and/or vehicle rollaway. The investigation found that the subject vehicles contain a logic that prevents the engagement of the park gear when the park gear request is made while the vehicle is still moving at low speeds. In the event the driver requests the park gear prior to the vehicle achieving a complete standstill status, the vehicle rejects the park gear request, displays a visual warning, and shifts into the neutral gear. Therefore, if the driver does not properly restrain the vehicle prior to exiting (i.e., allow the vehicle to come to a complete stop, then provide a second park gear request and/or engage the parking brake), the vehicle is at risk of freely rolling away in neutral. ODI found that existing strategies to deter drivers from exiting the vehicle after failing to ensure the transmission successfully engages the park gear have not stopped some from doing so. The absence of additional mechanisms to mitigate the effects of driver behavior in failing to ensure the park gear was engaged prior to exiting the vehicle constitutes a defect presenting a risk to motor vehicle safety. ODI is aware of one reported injury due to this safety defect. On November 15, 2024, Fisker filed recall 24V-865 for all MY 2023-2024 Fisker Ocean vehicles. The recall remedy is the release of software upgrade OS 2.0. This software update provides additional robustness to prevent unintended vehicle movement via two major changes. First, in the event the driver seatbelt is unlatched when the vehicle is traveling at low speeds, logic was added to initiate an automatic, controlled deceleration to bring the vehicle to a standstill. Once standstill status is achieved, the vehicle will automatically engage the park gear and the electronic parking brake. Second, OS 2.0 introduced an "Auto Vehicle Hold" feature, which holds the vehicle in standstill by maintaining friction brake pressure when the driver removes their foot from the brake pedal. This feature prevents the vehicle from rolling under gravity, which therefore helps engage both the park gear and electronic parking brake. In view of the recall action being taken by Fisker, ODI is closing this Preliminary Evaluation. The agency reserves the right to take additional action if warranted by future circumstances. On February 13, 2024, the Office of Defects Investigation (ODI) opened Preliminary Evaluation PE24004 to assess alleged failures of the rear-view camera (RVC) in 2020-2023 Model Year (MY) Honda Ridgeline vehicles. At this time, ODI has received a total of 50 complaints with allegations of the RVC image failing to display when the vehicle was in reverse. The RVC wire harness is routed near the hinge in the tailgate, in a location where frequent opening and closing of the tailgate causes bending stress on the harness. Honda determined the wire harness was manufactured with a material which is permeable to water and salt, which when it freezes and thaws, combined with tailgate usage, ultimately leads to breakage of the wiring in the tailgate harness. This results in a failure to display the RVC image. On May 9, 2024, Honda filed a Part 573 Safety Recall Report recalling all 2020-2024 MY Honda Ridgeline vehicles. The dealer will replace the RVC tailgate harness with a wire harness that is manufactured by a new supplier and has improved material properties to withstand wear from bending. In view of Honda filing recall 24V-321 addressing the subject of this investigation, ODI is closing this Preliminary Evaluation. The Office of Vehicle Safety Compliance (OVSC) tested two products marketed and sold by Nuna Baby Essentials, Inc. ("Nuna") under the model names "Nuna PIPA lite" and "Nuna PIPA lite r" to the requirements of Federal Motor Vehicle Safety Standard (FMVSS) No. 213, "Child Restraint Systems." As documented in each model's test report, the PIPA lite and PIPA lite r appear not to comply with a number of FMVSS No. 213 requirements, including but not limited to the means of installation in S5.3.2, and labeling requirements in S5.5. As each model's test report documents, the products' "infant car seat"'as it is referred to by Nuna in Nuna's printed instructions'did not appear capable of being installed using a Type I seat belt assembly. Further examination of Nuna's certification of the Nuna PIPA lite and Nuna PIPA lite r as compliant with FMVSS No. 213 is necessary. Through this investigation, the agency will obtain additional information about the basis for Nuna's certification of these products.

Summary
The Office of Defects Investigation (ODI) received a petition on February 2, 2024, requesting a defect investigation into an alleged defect of the "engine" on the following vehicle model: 2017 Hyundai Santa Fe vehicles equipped with the 3.3L V-Shaped GDI Lambda-II engines. The petition cites allegations of a loss of motive power, often resulting in an engine seizure. The stall is preceded by a knocking sound, decreased performance and no warning lights. An analysis of the subject vehicle population was undertaken using complaints submitted to ODI along with manufacturer data. It was determined that the frequency of events is low for the subject population, while those that specifically report a loss of motive power event have a high mileage at the time of incident. The 2017 Santa Fe is equipped with a service interval reminder for the consumer that must be set by the consumer themselves or at a dealership. A Technical Service Bulletin (19-EM-002H) instructs the dealership to set the maintenance reminder at the completion of routine maintenance service or anytime a repair is performed that requires new engine oil. This maintenance reminder will help reinforce the need for proper oil maintenance intervals. Additionally, the Santa Fe vehicles with 3.3L V-shaped Lambda-II engines will be added to Hyundai's existing Technical Service Bulletin (23-EM-007H) related to combustion chamber cleaning. These actions will help limit negative consumer incidents related to loss of motive power. Lastly, Hyundai has extended the limited engine warranty for MY 2013-2019 Santa Fe vehicles equipped with 3.3L V-shaped Lambda-II engines included in the Petition. The updated coverage is 15 years, or 150,000 miles and details are contained in Technical Service Bulletin 24-EM-003H. In view of the service interval reminder, combustion chamber cleaning, and extended warranty actions being taken by Hyundai, ODI is denying this Defect Petition. The agency reserves the right to take additional action if warranted by future circumstances. Summary
On January 11th, 2024, the Office of Defects Investigation (ODI) received Recall 24V-011 that identified a safety defect in the High Voltage Coolant Heater (HVCH) installed in certain Model Year (MY) 2022-2023 Lucid Air vehicles manufactured prior to July 2022. The HVCH may experience an internal delamination failure, which poses a risk of the vehicle losing defrost capability. If the HVCH fails, coolant cannot be heated, and the windshield cannot be defrosted. Loss of adequate windshield defrost capability could lead to restricted driver field of vision and an increased risk of a crash. The remedy outlined in this recall is a software update that identifies any HVCH failure when it occurs, which then provides a notification on the glass cockpit panel about the lack of defrost capability and advises the owner to contact Lucid for an HVCH replacement. On July 27th, 2023, Jaguar Land Rover North America, LLC filed Recall 23V-518 which affects Jaguar Land Rover vehicles equipped with the same HVCH as the Lucid Vehicles that were supplied by the same supplier (Webasto AG). Jaguar Land Rover North America, LLC's remedy is to replace the HVCH in the affected vehicles. The Office of Defects Investigation is concerned that by only providing a notification to drivers that their windshield defroster is not working, Lucid puts the driver in a situation where they could experience the failure during the same drive cycle as they receive the notification. In this scenario, the driver is not aware they have a failed HVCH until they are operating the vehicle and need the defrost function prior to being able to have the vehicle serviced. ODI is opening this Recall Query (RQ) to further assess the adequacy and safety consequences of the remedy described in Lucid Recall 24V-011.
