Summary
On March 7, 2024, the Office of Defects Investigation (ODI) opened a Preliminary Evaluation (PE24008) to assess reports of inadvertent activation of the Automatic Emergency Braking (AEB) system in model year 2019-2022 Honda Insight and 2019-2022 Honda Passport vehicles. The complainants allege activation of the AEB system with no apparent obstruction in the vehicle's path, resulting in rapid vehicle deceleration. Honda indicated that it is aware of a total of 412 reports that may relate to the alleged defect. Honda provided analysis of the alleged defect and stated that some customers possibly had an inadequate understanding of the AEB system and its limitations. However, many consumer complaints received by ODI allege that Honda dealerships were unable to reproduce the condition or state that Honda dealerships informed the consumer that this is considered normal AEB operation. To date, ODI has received a total of 106 consumer complaints of inadvertent activation of AEB in the subject vehicles.In total, ODI reviewed 475 reports involving vehicles with unique VINs that may relate to the alleged defect. Of the reports reviewed by ODI, three (3) allege a crash caused by the inadvertent activation of AEB and two (2) allege injury. This Preliminary Evaluation is being upgraded to an Engineering Analysis (EA25002) to further assess the scope, frequency, and potential safety related consequences of the inadvertent AEB activations. Further, the scope of the Engineering Analysis is being expanded to include assessment of model year 2023 Honda Passport vehicles. The Office of Defects Investigation (ODI) received a petition on February 20, 2024, requesting the opening of an investigation into an alleged defect of the "engine" on the following vehicle model stated by the petitioner: 2016 Kia Sedona vehicles equipped with the 3.3L V-Shaped GDI Lambda-II engine. The petitioner raised the issue of a Loss of Motive Power. On November 13, 2023, ODI opened PE23-019 to address allegations of loss of motive power, on the 3.3L V6 Lambda-II engine for 2016-2017 Kia Sorento and included the 2016 Kia Sedona as a peer vehicle. This investigation is ongoing and addresses the potential safety-related defect raised by the petitioner. In view of the open Investigation, PE23-019, ODI considers the petition moot and is closing this action. The closing of the petition does not constitute a finding by NHTSA that a safety-related defect does not exist. The Agency reserves the right to make that determination at the close of PE23-019.

A back up camera that does not display correctly can increase the likelihood of a crash or injury. On February 16, 2022, Ford Motor Company (Ford) issued a safety recall (NHTSA Recall 22V-089) for 2021-2022 Ford E-350 and E-450 vehicles equipped with dual rear wheels. The remedy involved inspecting the connection between the power steering pressure line and the hydraulic brake booster unit to prevent a sudden loss of power steering fluid, updating parts as necessary. This leak could result in a lack of power steering and power brake assist, increasing the risk of a crash. From July 2023 through December 2023, The Office of Defects Investigation (ODI) has received eleven consumer complaints alleging power steering line detachments after the vehicles had passed their inspection. The vehicles in the complaints include ambulances and a wheelchair lift-equipped school bus. In a meeting with Ford about the post-remedy failures on January 26, 2024, Ford informed ODI that they would file a new recall to update the power steering pressure line components on any vehicle that had not yet been inspected and also any vehicle which had passed inspection within the last twelve months. However, vehicles which had passed inspection more than twelve months ago would not be included within the scope of the new filing. ODI is opening this Recall Query (RQ) to evaluate the scope and remedy effectiveness of Ford's new recall (NHTSA Recall 24V-097). Summary
On July 22, 2022, the Office of Defects Investigation (ODI) opened investigation PE22-008 regarding complaints alleging a loss of motive power condition potentially due to a transmission malfunction in Model Years 2019-2021 Chrysler Pacifica Plug-In Hybrid Electric Vehicles (PHEV). On January 17, 2023, Fiat Chrysler Automobiles (FCA) issued safety recall (NHTSA Recall 23V-010) involving 2017-2023 Model Years (MY) Pacifica Plug-In Hybrid Electric Vehicles (PHEV) produced between August 12, 2016, through January 9, 2023. The recall states that the loss of motive power condition in these vehicles is caused by an internal transmission wiring connector that may short circuit, resulting in an unexpected engine shutdown. An unexpected engine shutdown can cause a loss of drive power, increasing the risk of a crash. In discussions with FCA on Recall 23V-010, ODI confirmed the recall was related to investigation PE22-008. The remedy for 23V-010 is to update the Power Inverter module (PIM) and/or Instrument Panel Cluster (IPC) software to provide drivers a message stating "Stop Safely - Vehicle Will Shut Off Soon" followed by a chime and MIL, PRNDL blinking (on cluster and shifter knob), low SOC (State of Charge) warning and reduced power mode. The recall remedy does not replace the defective hardware that was described in Recall 23V-010, and the wiring connector short still exists on these vehicles. To date, ODI has received 132 reports for the population of vehicles subject to recall 23V-010. These reports allege a loss of motive power condition that reduces the vehicles speed, followed by dashboard warning lamps displaying messages associated with recall 23V-010. The presence of these warning lamps and messages indicates the vehicle likely had the recall remedy applied. Follow up discussions with FCA confirm their remedy does not prevent a loss of motive power, but instead limits torque to the engine and after an unspecified number of miles or period of time, the engine will shut down. When this occurs, FCA relies on the owner to transport the vehicle to a dealership after they experience a loss of motive power condition, at which time the defective hardware is replaced. Furthermore, the dashboard messages introduced in the recall remedy may cause the driver to pull over in an unsafe location. Pulling over in an unsafe location may expose the occupants to undue harm. The Office of Defects Investigation is opening this Recall Query (RQ) to assess the adequacy, frequency, and safety consequence of the remedy for recall 23V-010.
