On March 22, 2012, Spartan Motors Chassis, Inc. (Spartan) notified the NHTSA that it had decided a defect exists in certain model year 2012 K2 and MG chassis manufactured from December 15, 2011, through January 31, 2012, and equipped with 3030 brake chambers.These vehicles fail to comply with the requirements of Federal Motor Vehicle Safety Standard (FMVSS) Number 121, Air Brake Systems because they do not meet the park brake and draw bar requirements.The park brake may not hold the vehicle in place and if the vehicle moves, injury or property damage could occur without warning.NHTSA assigned recall number 12V-125 to this matter.Spartan reported that the manufacturer of the noncompliant brake chambers was TSE Brakes, Inc. (TSE).On April 9, 2012, the Recall Management Division (RMD) mailed an information request (IR) letter to TSE requesting a list of purchasers who received the noncompliant brake chambers, and also inquiring if TSE sold any affected brake chambers in the replacement market.In a letter dated April 20, 2012, TSE responded to RMD?s IR letter by explaining their non-concurrence with Spartan?s noncompliance decision. TSE noted that, to date, Spartan has provided three draw bar test results.The first test, dated November 29, 2011, ?significantly failed? while two tests dated February 7, 2012, ?significantly passed.?TSE concluded that these results ?ruled out the TSE actuator as an explanation? due to the actuators displaying the same output force yet having different test results.Also, TSE made note that there is no FMVSS requirement for actuator performance as FMVSS 121 compliance only relates to vehicle braking performance.The actuators used on Spartan?s 13 recalled chassis, according to TSE, meet industry standards per SAE J2318.TSE noted that these brake chambers are ?produced in Mexico and shipped to the U.S. for Spartan Motors only.?Irrespective of the differing views between TSE and Spartan, all chassis that could have been impacted by the reported noncompliance have been recalled.This EQ is closed.

) submitted a Defect Information Report to NHTSA identifying a safety defect regarding fires originating between the transmission skid plate and the exhaust catalyst in approximately 65,246 model year (MY) 2010 Jeep
Wrangler 4x4 vehicles with automatic transmissions (NHTSA Recall 12V-216).According to Chrysler
, changes to the exhaust system for MY 2010 resulted in reduced clearances between the exhaust catalyst and the skid plate.If debris collects in the area of the skid plate and is not removed, there is an increased potential for underbody debris fires.ODI's analysis of skid plate area fires in the MY 2010 Wrangler vehicles with automatic transmissions found that the rate increased significantly from approximately 1.7 fires per 100,000 vehicle years in the MY 2007 through 2009 vehicles with the prior exhaust design to approximately 14.8 fires per 100,000 vehicle years in MY 2010 vehicles, a nearly 800 percent increase.In analysis of both Vehicle Owner Questionnaire (VOQ) data submitted to the Office of Defects Investigation (ODI) and Chrysler
's consumer complaint data submitted to ODI, 35 unique reports were identified by Chrysler
alleging fire in subject vehicles with origins near the engine compartment or in the underbody area directly beneath and to the rear of the engine compartment.Chrysler
's assessments of the 35 reports of fire were categorized by likely causes.Eleven of the fires were determined to be caused by skid plate debris accumulation and 4 additional fires were found to have originated in the area of the skid plate, but debris ignition could not be confirmed.An additional skid plate debris fire was reported to ODI after the recall announcement, bringing the total known fire count to 36, including 12 caused by skid plate debris and 4 originating in the skid plate area from unknown causes.Chrysler
identified a design change to the exhaust system in MY 2010 that increased the possibility of debris contact with an exhaust catalyst.Beginning in MY 2011, the skid plate was replaced with a skid bar for weight reduction purposes.The skid bar having a much smaller surface area than the skid plate reduces the potential for debris entrapment.The recall remedy for recall 12V-216 will involve replacement of the skid plate with the skid bar.As background, the MY 2010 subject vehicles are a part of the JK generation of Jeep
Wrangler vehicles that began production in MY 2007.Chrysler
conducted a prior recall (09V-436) of MY 2007 and 2008 Jeep
Wranglers for a defect involving overheated transmission fluid expulsion that could contact the exhaust and cause an engine compartment fire.The recall remedy was to add audible (chime) and visual warnings (hot oil message in the instrument cluster) to alert the driver that the vehicle was being operated with elevated transmission fluid temperatures.Chrysler
enhanced the audible warning with a service campaign (J31) for MY 2009 and a portion of the subject MY 2010 vehicles.Transmission fluid expulsion has not been identified as the cause of any of the 36 fire incidents in the subject vehicles. No indications of a common causal pattern were evident in the remaining 20 fire incidents in the subject vehicles.Six (6) of the incidents appear to be related to improper wiring of various added aftermarket equipment, 2 appear to be related to improper oil change, 1 was related to a damaged transfer case, 1 was related to an electrical short circuit and the remaining 10 were of unknown origin.Accordingly, this Preliminary Evaluation is closed.The ODI reports cited above can be reviewed at www-odi.

In a June 21, 2012 letter to NHTSA, General Motors LLC. (GM) submitted a Defect Information Report (DIR) identifying a defect in approximately 413,148 model year (MY) 2011 through 2012 Chevrolet
Cruze vehicles manufactured from October 2, 2009 through May 31, 2012 (Recall 12V-288). According to GM, engine oil change procedures can result in spilling or dripping of oil on the engine shield.Also, manual transmission vehicles with a completely worn clutch may cause hydraulic fluid to be expelled from the clutch housing vent hole.If oil, debris or the burning hydraulic fluid contacts hot engine or exhaust system surfaces, and the engine shield, the shield may ignite and burn, leading to a possible engine compartment or vehicle fire.GM and its engine shield supplier, Hanwha, modified the engine shield to address factors that may contribute to engine compartment fire. The redesigned shields were completely implemented in production vehicles by June 2012.The Office of Defects Investigation (ODI) analyzed complaint data provided by GM as well as complaints submitted to ODI from consumers. In total, there were 25 unique Chevrolet
Cruze reports indicating hot surface ignition due to engine oil or hydraulic fluid contacting hot manifold, exhaust surfaces and the engine shield.Four additional fires were identified that resulted from other or unknown causes, including 1 attributed to a missing oil filler cap.Three fires of unknown cause all involved vehicles equipped with 1.4L L4 turbocharged engines.ODI will continue to monitor field experience in Cruze vehicles equipped with that engine.Owners of the recalled vehicles are notified to take their vehicle to a GM dealer, who will modify the engine shield to prevent oil fluids from contacting the shield.Also, on vehicles with a manual transmission, dealers are to apply a protective tape to the electronic power steering wire harness to prevent any incidents related to this issue. ODI became aware of three bus fires that originated in the area of the transmission on the subject vehicles through media reports.Inspection of the vehicles indicated that the static parking brake, which is located behind the transmission, may have been applied as a result of an air leak while the vehicles were in motion.Under certain driving conditions, the parking brake can apply without the audible driver warning engaging.Parking brake application while the vehicle is in motion may result in a fire.To address the concerns of this investigation, Navistar has issued a recall (12V-479) that will add a switch to detect parking brake lever movement while the vehicle is in motion (in addition to the existing switch that monitors brake line air pressure) and set an additional warning light to warn the driver of application of the parking brake.Navistar
