During communications with Government Services Administration (GSA) staff, the Office of Defects Investigation (ODI) was made aware of a 2012 Ford
F350 utility truck vehicle fire.The fire allegedly resulted from high exhaust temperatures igniting materials within the passenger side, lower rear storage compartment of the Manning utility body.As a result of this incident, an equipment alert (USFS-EA-2012-01) was issued by GSA to operators of GSA-owned fleet vehicles equipped with the subject Manning utility body instructing them not to store flammable liquids or combustible materials in the subject storage compartment.The San Dimas Technology and Development Center (SDTDC) was contracted by GSA to conduct testing of the Manning utility body to determine the cause of the fire.Testing performed by SDTDC engineers found temperatures in the subject utility body compartment well above those of peer vehicles.The elevated temperatures were found to be the result of the exhaust pipe routing configuration that directed exhaust gases onto the utility body undercarriage.Furthermore, above average temperatures were found to melt plastic and rubber parts located just below the exhaust outlet.By its letter dated August 21, 2013, Manning Equipment Inc. LLC notified ODI that it initiated a safety recall (NHTSA Recall No. 13V-380) to address a safety defect in 37 model year (MY) 2012 Ford F-Series gas, dual rear wheel chassis cabs equipped with Manning Equipment installed service bodies. The affected vehicles were manufactured from October 2011 through June 2012.Manning has informed ODI they will remedy the subject vehicles with a tailpipe extension that will direct exhaust out and away from rear compartment and a decal placed above the compartment instructing users not to store flammable liquids or combustible materials in the subject compartment.Based on Manning?s action to remedy the subject vehicles for the defective exhaust pipe design, this investigation is closed.

In a letter dated August 9, 2013, Hyundai-Kia America Technical Center, Inc. (Hyundai) notified NHTSA that Hyundaiis conducting a safety recall (13V-354) involving approximately 239,000 vehicles involving model year (MY) 2006 through 2010 Hyundai Sonata vehicles (approximately 215,000)produced beginning on March 1, 2005 through January 21, 2010 and model year (MY) 2006 through 2011 Hyundai Azera vehicles (approximately 24,000) produced beginning on September 27, 2005 through November 22, 2010 that are registered in and operated or originally sold in the following 20 salt belt states and the District of Columbia: Connecticut, Delaware, Illinois, Indiana, Iowa, Maine, Maryland, Massachusetts, Michigan, Minnesota, Missouri, New Hampshire, New Jersey, New York, Ohio, Pennsylvania, Rhode Island, Vermont, West Virginia, and Wisconsin.Hyundai will also inform dealers to inspect/repair vehicles that were previously registered in salt belt states but are currently registered elsewhere. Hyundai is conducting the recall to remedy a defect involving internal corrosion of the rear sub frame that may lead to separation of the rear upper/lower control arm(s) at their respective mounting points and/or separation of the axle from the transaxle where they meet.This could result in a loss of vehicle control and/or vehicle disablement which may increase the risk of a vehicle crash.In response to NHTSA?s Information Request (IR) letter, Hyundai stated the Azera vehicles utilizes the identical cross member and upper/lower rear control arm components as the subject (Sonata) vehicles.In analysis of both Vehicle Owner Questionnaire (VOQ) data submitted to the Office of Defects Investigation (ODI) and consumer complaint data submitted to Hyundai, ODI identified 19 non-duplicative reports alleging excessive corrosion of the rear sub frame. All of the complaint vehicles were either originally sold or registered at the time of the incident in one of the salt belt states.Of these 19 reports, 9 alleged either a suspension component separation or a fracture of the vehicle's rear cross member assembly.Similarly, ODI identified 8 warranty claims of rear sub frame replacement in data submitted by Hyundai due to excessive corrosion, all of which involved vehicles originally sold or operated in a salt belt state. On January 3, 2013, NHTSA received a letter (ODI No. 10487746) from Ms. Jessie A. Powell petitioning the agency to investigate drivability and braking concerns in the subject vehicle.The petition expressed two concerns: 1. The first software problem was when the vehicle shifted from battery to motor and caused such impact, I initially believed the vehicle had been struck in the rear.2. The next more alarming problem was NO BRAKES. The brake pedal traveled to the floor and a dashboard warning light flashed.This symptom occurred twice, leading to the vehicle being towed to the dealership, the second time in the dealership parking lot after diagnostics of the first incident.After a test drive and vehicle inspection, no actionable problem was found within the petitioner?s vehicle.The braking concern reported turned out to be normal vehicle operation.The petitioner identified other complaints of poor braking performance and low brake pedal received by NHTSA concerning Prius models. Broader review of the consumer complaints reported for all variants of the subject vehicle showed no indication that either the reported jolting sensation or the brake performance concerns reported are occurring in this vehicle population at a level that would require investigative action by NHTSA.The following recalls by Toyota were to address many of these complaints in Prius variants. Neither of these recalls is applicable to Ms. Powell?s 2012 Prius C: 1. Recall 10V-039 March 5, 2010 - 2010 Prius, Lexus HS - Reprogramming ABS ECU - Improve Antilock brake function over bumpy surfaces. 2. Recall 13V-235 August 7, 2013 - 2010 Prius, Lexus HS - Replace Brake Booster / Pump assembly - Low brake pedal due to nitrogen bubble in hydraulic portion of brake system. In the Agency?s view, additional investigation is unlikely to result in a finding that a defect related to motor vehicle safety exists in the subject vehicles.Therefore, given the need to allocate and prioritize NHTSA limited resources to best accomplish the Agency?s safety mission, the petition is denied.This action does not constitute a finding by NHTSA that a safety-related defect does not exist.The Agency will take further action if warranted by future circumstances.More details concerning our review can be found in the Federal Register Notice for this action.

Through regular communications with heavy truck fleets, ODI learned of a potential issue involving failures of both the windshield wiper motor or linkage on model year 2011-2012 Volvo VNL trucks.ODI searched its Early Warning Reporting (EWR) database and identified a significant number of claims of wiper system problems reported by Volvo.In some cases the windshield wiper linkage separated and in others the wiper motor failed.In both failure modes, the visibility of the driver is significantly diminished.In response to an ODI Information Request (IR) dated 25 March 2013, Volvo provided warranty data that identified three distinct windshield wiper failure modes on the subject vehicles: wiper motor, wiper linkage, and wiper arms.Analyzing the warranty data revealed elevated rates of wiper motor and wiper arm failures.The warranty data further showed that the wiper arm failures occurred early in the vehicle service life suggesting that those that were likely to fail had already doneso.However the wiper motor failures were found to be an ongoing concern.Owners continued to file warranty claims at an elevated rate at the time of this investigation.Following a meeting on 25 July 2013, Volvo notified ODI on 9 August 2013 they had determined the wiper motor drive gear may prematurely fail on subject vehicles manufactured from 1 February 2010 through 30 September 2010 and 1 June 2011 through 30 November 2011.Volvo states that the failure presents a risk of decreased visibility through the windshield, which may result in a vehicle crash.Volvo initiated a safety recall (NHTSA Recall No. 13V-349) to address the safety defect in approximately 17,824 Model Year 2011 and 2012 VAH, VHD, VNL, and VNM Trucks.Volvo?s action to remedy the subject vehicles for the defective wiper motor resolves the issue raised by this investigation and therefore this investigation is closed.

ODI was made aware of multiple allegations of engine stall resulting from a loose barcode sticker in the fuel tank of certain model year (MY) 2012 Thomas Built Saf-T-Liner transit style buses manufactured by Thomas Built Buses, Inc. (TBB).The loose barcode sticker from a diesel powered cab heater (installed in some cold weather state buses) may become lodged in the fuel delivery pickup tube and restrict fuel flow. Early analysis suggested that at least one failure resulted in a school bus being stranded on an interstate highway.Additionally, ODI identified 6 Early Warning Reporting (EWR) reports that appear related to the alleged defect, and TBB informed ODI that it was aware of nine (9) other related field reports for a total of fifteen (15) alleged incidents.Daimler Truck North America (DTNA), parent company of TBB, submitted a recall notice dated August 13, 2013 (13V-365 for school buses and 13V-364 for non-school buses) that addresses ODI's concerns.Fuel tanks will be inspected for the presence of a label.If a label is found, it will be removed from the tank.As a result of the DTNA remedial action this investigation is closed.
