The dealer confirmed the tread separation and informed the contact that there was a crack on the front tire. The dealer recommended that both tires be replaced. The vehicle remained with the dealer unrepaired. The manufacturer was notified and was coordinating with the tire manufacturer regarding the potential tire replacements. Summary
Both left and right CV axles make loud clicking, grinding noise when turning. I reported this issue to Honda at around 6,500 miles. At first I was told we are waiting for a solution. Now the dealership's service manager says that there is no safety issue and that Honda has no intention of fixing the problem. The car currently has less than 16,000 miles and the clicking just keeps getting louder. It doesn't feel safe and at the very least it's annoying and unacceptable.

Multiple vehicle systems have malfunctioned, including the transmission, infotainment/display screen, and backup camera. Most recently, the backup camera has stopped functioning, preventing a rear view while reversing. The vehicle is available for inspection upon request, along with any service records documenting prior repair attempts.The transmission issues created an unsafe driving condition due to unpredictable shifting and hesitation. In addition, the backup camera is no longer functioning, significantly reducing rear visibility while reversing and increasing the risk of striking a pedestrian, child, vehicle, or object. These defects pose a safety risk to both the driver and others around the vehicle.The vehicle has been presented to an authorized Toyota dealership on multiple occasions for repair of various issues. Most recently, I sought service for the inoperative backup camera but was refused service. Previous concerns have been documented through dealership service visits.The vehicle has been inspected by an authorized Toyota dealership during previous repair attempts. It has not been inspected by police or insurance representatives.The vehicle experienced ongoing drivability and infotainment issues before the backup camera failed. The backup camera eventually stopped functioning entirely.

I am submitting this complaint to request that the National Highway Traffic Safety Administration (NHTSA) document and evaluate what may be a broader defect affecting certain Toyota Sienna vehicles and determine whether it presents a potential safety-related defect trend. My 2015 Toyota Sienna was diagnosed by an authorized Toyota dealership with an external rear-bank head gasket leak. A head gasket is a critical engine component, and failure can result in coolant loss, engine overheating, reduced engine performance, stalling, or complete vehicle disablement. These conditions may create a safety risk, particularly if they occur while driving in traffic or at highway speeds. An authorized Toyota dealership confirmed the diagnosis and submitted a goodwill assistance request to Toyota. The dealership noted my history as a repeat Toyota customer and recommended that assistance be provided. Toyota denied the goodwill request based on the vehicle's age and mileage. I subsequently appealed through Toyota's customer relations process, and the denial was upheld. I understand that NHTSA does not resolve individual warranty or customer-assistance disputes. My purpose in submitting this complaint is to request that NHTSA determine whether similar reports exist involving premature head gasket failures in this model year or related vehicles and, if warranted, evaluate whether a broader defect trend exists that could present a safety concern. Potential safety consequences include engine overheating, loss of power, stalling, and unexpected vehicle disablement. The vehicle has been maintained in accordance with manufacturer recommendations and has not experienced any overheating event that would reasonably explain a premature head gasket failure. My vehicle has experienced repeated unexpected engine shutdown/no-start events, creating a significant safety hazard. The engine has unexpectedly shut off or failed to restart while stopped in traffic and while entering traffic, resulting in a sudden loss of propulsion. On one occasion, the vehicle would not restart for approximately one minute. The vehicle was inspected by an authorized Honda dealership, and the starter was replaced under warranty in July 2025. The repair did not correct the problem. Since that repair, the same failure has occurred multiple additional times. The issue has now occurred at least six times over approximately one year. Because the condition is intermittent, the dealership has not permanently identified the root cause. The vehicle is currently being returned to the dealership for further diagnosis, and I have requested that the matter be escalated to Honda Technical Assistance (Tech Line) and Honda Engineering. This defect creates a serious safety concern because the vehicle can unexpectedly lose power while in or entering traffic, increasing the risk of a collision. There were no consistent warning lights or messages before the failures, and the condition occurs intermittently without warning. The vehicle remains available for inspection.
