Reason For Notice
Our records indicate that we shipped you the above product that does not conform to our standards. The color of
the units is blue and not yellow. This is not a safety concern.
1. Please identify inventory of part numbers 59417GM, 59029GM, 59603GM, 55609GM, 59589GM,
59371GM and 59414GM with the packaging date codes prior AH25211.
Pictures of the location of the date codes can be found below.
2. Please scrap affected units and use the affidavit form included
3. Email a copy of the completed affidavit to [email protected], referencing RGA 32116 in the subject line.
4. Credit will be issued to your account upon receipt of affidavit form.
