Root Cause:
e Valve crosshead contact with internal actuator wiring harness injector wire causing fatigue
near crimp connector
Verification
Inspect internal actuator wiring harness for fractured or damaged areas and/or contact with the
valve crosshead as seen in Figure 1. As a result of contact with the valve crosshead, the internal
actuator wiring harness damage is not always obvious. Check for signs of damage to the wiring
insulation near the crimp connector. See Figure 2.
