Summary
The Office of Defects Investigation (ODI) has received twenty-three complaints alleging excessive frame flex on model year 2017-2023 Grand Design fifth wheel recreational trailers, models Momentum and Solitude.Twenty-two of the complaints allege that excessive frame flex resulted in a cargo or entry door opening while in transit, some with objects lost on roadways, while six complaints also state that slideouts extended out partially. ODI has been in contact with both Grand Design and Lippert, the frame manufacturer. Grand Design believes that some frame flex, generally considered to be vertical movement in the upper deck area of the fifth wheel, is normal. However, Grand Design has developed a technical service bulletin (TSB), to evaluate concerns of excessive frame flex, defined as vertical movement greater than 3/8 inch. Per Grand Design, the frame flex is only occurring in the upper deck area of the fifth wheel and is resulting in cosmetic defects such as moving sidewalls, damage to cabinets, binding doors, etc. However, Lippert believes that the effects of frame flex may also extend back to the front axle of the trailer.While Lippert provides frames for multiple trailer manufacturers, the majority of complaints are for these Grand Design products. Cargo and entry doors that open and slideouts that extend while in transit increase the risk of injury or a crash. ODI is opening this Preliminary Evaluation (PE) to evaluate the severity of the potential problem and to determine whether an unreasonable risk to motor vehicle safety exists. The Office of Defects Investigation (ODI) has received 36 complaints alleging a loss of braking ability in model year 2015-2017 Ford Edge vehicles manufactured by Ford Motor Company. The complaints allege a loss of braking ability that occurs while braking and is the result of a rear brake hose failure. The complaints also allege that the loss of braking ability occurs without any prior warning, and results in an increase in stopping distance. Some complaints also allege previous brake repairs prior to the failure, some referencing NHTSA Recall 20V-469. ODI is opening this Preliminary Evaluation (PE) to determine the scope and severity of the alleged problem, and to fully assess the potential safety-related consequences. The Office of Defects Investigation (ODI) has received six complaints (VOQs) alleging failures of Enel X Way's JuiceBox Level 2 residential EV chargers. One individual's complaint alleges that charging their vehicle with a JuiceBox led to an explosion in their garage that set fire to a home and rendered it a total loss. Another individual alleges that a car caught fire in their driveway while charging with a JuiceBox. Four other complaints describe electrical malfunctions tripping circuit breakers and disrupting electric vehicle charge cycles. Additionally, ODI has become aware of a communication sent to charger owners on October 2, 2024 and posted on Enel X Way's website that Enel X Way North America will cease operations in the United States and Canada on October 11, 2024. Ceasing operations could also impact the charging app and supporting software. ODI has opened a Preliminary Evaluation to assess the scope, frequency, and safety consequences of the potential safety-related problem, including the safety impacts related to the app and supporting software. On April 18, 2024, the Office of Defects Investigation (ODI) received a Defect Petition requesting the National Highway Traffic Safety Administration (NHTSA) investigate the Nuna Rava child seat for a harness that can allegedly loosen without pressing the harness release button. The Petitioner states there are 24 complaints in the NHTSA database all describing a similar issue.According to the Petitioner, "this condition has poor detectability and the consequence of failure in case of a crash would be severe or fatal injury". On April 25, 2024, ODI received a second Defect Petition requesting NHTSA investigate Nuna child seats for a similar allegation as the initial petitioner. Upon receiving these Defect Petitions, ODI sent an Information Request (IR) letter to Nuna on May 16, 2024, asking for information regarding the petitioners' allegations. Nuna requested a two-week extension and responded to ODI's IR letter on June 21,2024. On July 18, 2024, Nuna submitted a supplemental response to ODI's IR Letter. After evaluating the petitioner's allegations and Nuna's response to ODI's IR letter, ODI has decided to grant the petition and open this defect investigation.

The Office of Defects Investigation has identified 14 Vehicle Owner Questionnaire (VOQ) reports on 2023 through 2024 model year (MY) VinFast VF8 vehicles alleging problems with the Lane Keep Assist (LKA) system. The complaints allege the system can activate in error and not function properly. The VOQs allege the system has difficulty detecting lanes on the roadway, provides improper steering inputs, and is difficult to override by the driver. Improper steering inputs can result in high control effort required by the driver and increase the likelihood of a crash or injury. ODI is opening this Preliminary Evaluation (PE) to assess the scope, frequency, and severity of the potential problem and to determine if a safety related defect exists in the subject vehicles.
