On August 13, 2021, NHTSA?s Office of Defects Investigation (ODI) opened a Preliminary Evaluation (PE21-020) to assess the performance of TeslaeBay?s Autopilot system (a system characterized by TeslaeBay as an SAE Level 2 driving automation system designed to support and assist the driver in performing the driving task) available in TeslaeBay vehicles. The investigation opening was motivated by an accumulation of crashes in which TeslaeBay vehicles, operating with Autopilot engaged, struck stationary in-road or roadside first responder vehicles tending to pre-existing collision scenes. Upon opening the investigation, NHTSA indicated that the PE would also evaluate additional similar circumstance crashes of TeslaeBay vehicles operating with Autopilot engaged, as well as assess the technologies and methods used to monitor, assist, and enforce the driver?s engagement with the dynamic driving task during Autopilot operation.PE21-020 is upgraded to an Engineering Analysis (EA) to extend the existing crash analysis, evaluate additional data sets, perform vehicle evaluations, and to explore the degree to which Autopilot and associated TeslaeBay systems may exacerbate human factors or behavioral safety risks by undermining the effectiveness of the driver?s supervision. Summary Description: Rearview Camera FMVSS 111 Noncompliance


Summary

On September 23, 2020, FordeBay Motor Company (FordeBay) filed a noncompliance recall report under 49 CFR Part 573 (NHTSA Recall No. 20V-575), recalling 620,246 Model Year (MY) 2020 FordeBay F-150, F-250, F-350, F-450, F-550, Explorer, MustangeBay, Transit, Expedition, Escape, Ranger and Edge, and LincolneBay Nautilus and Corsair vehicles for a noncompliance with Federal Motor Vehicle Safety Standard (FMVSS) 111, Rear Visibility. FordeBay's recall described the noncompliance as insufficient electrical conductivity within the printed circuit board (PCB) internal to the camera leading to intermittent rearview camera operation. The supplier of the cameras was Magna Electronics.According to FordeBay's chronology in the Part 573, during the period February-April 2020, FordeBay identified and monitored warranty claims pertaining to intermittent or inoperative rearview cameras. FordeBay reviewed supplier product changes, focusing on changes introduced by the supplier in the October-November 2019 time frame. The Part 573 stated that the issue was brought to FordeBay's Critical Concerns Review Group (CCRG) for review on May 12, 2020.On July 7, 2020, NHTSA's Vehicle Research and Testing Center (VRTC) informed the Office of Defects Investigation (ODI) of a trend of MY 2020 Escape vehicles with rearview camera malfunctions. ODI contacted FordeBay the next day, providing five Vehicle Owner Questionnaires (VOQs) alleging inoperative rearview cameras. On July 15, 2020, FordeBay met with NHTSA to discuss the rearview camera issue. In a subsequent August 24, 2020 meeting, ODI and NHTSA's Office of Vehicle Safety Compliance (OVSC) met with FordeBay to discuss the recall status and additional VOQs. The Office of Defects Investigation (ODI) of the National Highway Traffic Safety Administration (NHTSA) opened this PE after receiving 11 Vehicle Owner Questionnaires (VOQs) alleging that Model Year (MY) 2019 Mercedes-BenzeBay (MB) SprintereBay vans, configured for use as Amazon delivery vehicles or ambulances, roll away shortly after being shifted to park using the Auto-P function. A rollaway vehicle with no operator behind the steering wheel could potentially strike pedestrians, moving or parked vehicles or buildings, resulting in injury, fatality and/or property damage. Analysis of data provided by MB in response to the ODI Information Request (IR) letter showed a high failure rate. Following discussions with ODI about the failures, MB decided to conduct a safety recall. On December 15, 2021, Daimler VanseBay USA, LLC filed recall 21V-972 covering 48,400 model year (MY) 2019-2020 Mercedes-BenzeBay (MB) SprintereBay vans. With recall 21V-972, this investigation is closed. The affected vehicle population determined by MB is SprintereBay vehicles (VS30, Platform 907) produced between June 5, 2018 and November 30, 2020 with a manual parking brake and a 7-speed automatic transmission that was manufactured between May 1, 2018 and October 31, 2019, in the Hedelfingen transmission plant in Germany.Further use of Agency resources does not appear to be warranted. The closing of this investigation does not constitute a finding by NHTSA that a safety-related defect does not exist on other model or MY vehicles outside of the recall scope. The Office of Defects Investigation (ODI) has opened this Preliminary Evaluation (PE) on HaldexeBay Gold Seal GC3030LCW air brake chambers installed in model year (MY) 2015-2020 vehicles based on allegations of air loss resulting in vehicle fires. ODI is aware of eleven (11) Vehicle Owner Questionnaires (VOQs) that allege, without discernable warning to the driver, the failure of a HaldexeBay brake chamber. Seven (7) of those cases involved thermal events causing extensive fire damage to the vehicle and, in some cases, the cargo too. In at least one of the fires, a fire investigation report identified a wheel end fire as the origin. Analysis of other failed units found HaldexeBay brake chamber diaphragms punctured by the power spring.ODI discussed the VOQs with the complainants. Through these discussions, ODI learned that multiple fleets were having issues with numerous vehicles. Summary Description: Sliding Doors Inoperative


Summary

The Office of Defects Investigation (ODI) opened PE21-016 on July 7, 2021, to investigate 364 complaints alleging that one or both sliding doors on 2016 Model Year (MY) Grand Caravan or Town and Country vehicles could not be opened. The door(s) could not be opened using the mechanical handle or electronic remote switches. Many of the complaints described having to remove passengers seated in the rear of vehicles through the front doors, the second sliding door (if it was operational), the window, or the car?s rear hatch. The complainants have expressed concerns that if the sliding door(s) cannot be opened in the event of an emergency or crash, it could trap passengers or delay their egress.On June 4, FCAeBay published TSB# 23-017-20 instructing dealers to replace the door lock actuator instead of the entire door latch assembly on the subject vehicles when consumers complained about either one or both of the sliding door locks not functioning and/or emitting a loud buzzing noise during lock/unlock operation. On June 16, 2021, FCAeBay approved a Quality Warranty Extension for fifteen years/150,000 miles from the vehicle's in-service date for sliding door lock actuators on vehicles built between May 1, 2015, through January 9, 2017. FCAeBay has started notifying owners about the new program. FCAeBay will provide reimbursement for owners who previously paid for sliding door repairs.Consumers whose sliding doors are inoperative will have the opportunity to have their vehicle diagnosed, and if a fault is identified, have the sliding door actuator repaired at no charge.Analysis of the FCAeBay IR response and ODI complaint data found that the majority (96%) of sliding door failures occurred prior to the calendar year 2020 when the subject vehicles had approximately 4 years in service. At the time of the closing of this investigation, 99% of the subject vehicles are passed the age where failures typically occur. The number of new reports has been decreasing since 2020 and further reduced following the FCAeBay Warranty Extension program. ODI found that prior to a failure of the sliding door(s) there is normally a loud buzzing noise/sound generated that alerts the operator to the actuator starting to wear out.ODI has not identified any crashes or injury allegations due to the sliding door being inoperable on the subject vehicles following nearly six years of exposure. ODI will continue to monitor complaints received for the incidence of sliding doors becoming inoperative.Accordingly, the investigation is closed.
