The Office of Defects Investigation (ODI) has received 3 complaints alleging inadvertent deployment of the seat belt retractor pretensioner in model year 2019-2020 Ford Expedition vehicles manufactured by Ford Motor Company. The complaints allege hearing a loud sound immediately followed by the seat belt rapidly tightening. The complaints do not allege a crash or impact occurred that may have resulted in the deployment of the pretensioner system. Additionally, the seat belt remains in a locked position after the deployment event. On February 9, 2024, Ford issued safety recall 24V-099 which included 2018-2020 Ford Expedition and 2018-2020 Lincoln Navigator vehicles equipped with driver and front passenger seat belt retractor pretensioners built between October 1, 2018, and June 30, 2019. This recall addressed a defective seat belt retractor pretensioner assembly that may develop corroded squib pins, resulting in high resistance or open circuit. This condition may result in retractor pretensioner inadvertent deployment and increased risk of injury due to loss of pretensioner protection and a seat belt that is locked in position. The complaints allege not being included in recall 24V-099 but experiencing the failure that it addressed. ODI is also aware of additional reports of alleged pretensioner inadvertent deployment in 2019-2020 Ford Expedition and 2018-2019 Lincoln Navigator vehicles produced outside the dates defined by 24V-099. All 173 VOQs display failures that have characteristics consistent with those addressed in recall 23V-751 but are out of the scope of the recall. The Office of Defects Investigation (ODI) is aware of Electric Vehicle Distribution Module (EVDM) connector plate separation on Model Year (MY) 2020-2025 Freightliner eCascadia and MY 2022-2025 eM2 electric trucks. The EVDM connector plate contains multiple safety-related circuits including ABS, stability control and vehicle protective mechanisms to de-energize the High Voltage system. Separation of the EVDM connector plate can result in a loss of propulsion and increase the risk of a crash. An ODI review of Early Warning Reporting (EWR) submissions by Daimler Truck North America (DTNA) identifiedvehicles describing Controller Area Network (CAN) and power faults caused by EVDM connector plate separation. To better understand the severity and frequency of EVDM connector plate separation on the subject vehicles, a Preliminary Evaluation (PE) is being opened.

The Office of Defects Investigation (ODI) has identified four Standing General Order (SGO) reports in which a Tesla vehicle experienced a crash after entering an area of reduced roadway visibility conditions with FSD -Beta or FSD -Supervised (collectively, FSD) engaged. In these crashes, the reduced roadway visibility arose from conditions such as sun glare, fog, or airborne dust. In one of the crashes, the Tesla vehicle fatally struck a pedestrian. One additional crash in these conditions involved a reported injury. The four SGO crash reports are listed at the end of this summary by SGO number. ODI has opened a Preliminary Evaluation of FSD (a system labeled by Tesla as a partial driving automation system),which is optionally available in the Model Year (MY) 2016-2024 Models S and X, 2017-2024 Model 3, 2020-2024 Model Y, and 2023-2024 Cybertruck. This Preliminary Evaluation is opened to assess: -The ability of FSD's engineering controls to detect and respond appropriately to reduced roadway visibility conditions; -Whether any other similar FSD crashes have occurred in reduced roadway visibility conditions and, if so, the contributing circumstances for those crashes; and -Any updates or modifications from Tesla to the FSD system that may affect the performance of FSD in reduced roadway visibility conditions. In particular, this review will assess the timing, purpose, and capabilities of any such updates, as well as Tesla's assessment of their safety impact.
