Summary
On September 15, 2023, the Office of Defect Investigation (ODI) opened this Recall Query (RQ) after receiving 98 consumer complaints alleging loss of motive power in model year (MY) 2012-2018 Ford Focus vehicles. The RQ focused on the effectiveness of the remedies of Recall 18V-735 and 19V-515. In October 2018, Ford issued Recall 18V-735, which updated Powertrain Control Module (PCM) software in MY 2012-2018 Ford Focus vehicles equipped with 2.0L GDI engines built prior to April 13, 2017, and those with 2.0L GTDI engines built prior to February 2, 2018. The recall addressed vehicles experiencing a malfunctioning Canister Purge Valve (CPV) that can stick open and PCM software that does not adequately detect a stuck-open CPV condition. A CPV that is stuck open can cause excessive vacuum in the fuel vapor management system, potentially deforming the fuel tank, collapsing on itself. This deformation of the fuel tank can cause a Malfunction Indicator Light (MIL) to illuminate, an inaccurate or erratic fuel gauge indication, drivability issues, and/or loss of motive power. In July 2019, Ford issued Recall 19V-515, which addressed certain MY 2012 and MY 2017 Ford Focus vehicles equipped with 2.0L GDI and MY 2013-2014 Ford Focus vehicles equipped with 2.0L GTDI engines whose PCM software had been reprogrammed as a part of recall 18V-735. The updated software calibrations for these subject vehicles were not properly installed by Ford at the time of production. Approximately one year after this RQ was opened, on September 25, 2024, Ford initiated a customer satisfaction program (24N07) for MY 2012-2018 Ford Focus vehicles equipped with the 2.0L GDI and GTDI engines to'as ODI learned during this investigation'address a different issue with the CPV from the primary focus of the safety recalls (18V-735 and 19V-515). Ford stated the CPV may not operate as intended in these vehicles. A failed CPV on its own merit, may cause drivability concerns such as engine hesitation, running rough, or stalling at low speed, which will be accompanied by an illuminated MIL. This program provides a one-time replacement of the CPV for vehicles with 15 years of service or less (with no mileage restriction). This coverage will automatically transfer to subsequent owners. It is to be noted that the customer satisfaction program being taken by Ford to address this RQ investigation results in a much different and significantly less severe problem than the safety action taken to remedy 18V-735 and 19V-515. ODI found no further issues with the combined scope and severity related to 18V-735 and 19V-515 therefore, ODI is closing this Recall Query. The agency reserves the right to take additional action if warranted by future circumstances. On July 2, 2020, Kia America Inc. (Kia) filed safety recall 20V-389 for model year (MY) 2015-2016 Kia Soul EV vehicles. This recall was in response to allegations of unintended vehicle movement while in Park and provided updated electronic control unit software for the electronic parking brake logic as the remedy. The updated logic, which was included during production for MY 2017-2019 Kia Soul EV vehicles, automatically activates the electronic parking brake when the vehicle is powered off while the gear shift lever is in the Park position. On September 10, 2023, the Office of Defects Investigation (ODI) opened a Recall Query (RQ23-005) to investigate allegations of unintended vehicle movement while in Park in MY 2015-2019 Kia Soul EV vehicles. At the time that the investigation was opened, ODI had received 3 Vehicle Owner Questionnaires (VOQs) and additional Early Warning Reporting data involving reports of unintended vehicle movement in MY 2017-2018 Kia Soul EV vehicles. One of the VOQs submitted to ODI reported a rollaway collision with a fire hydrant, which occurred after the operator had parked and exited the vehicle. Following the opening of this Recall Query, ODI and Kia participated in a joint inspection the vehicle which had been involved in a rollaway collision. During the inspection, the subject vehicle exhibited the alleged defect, rolling both forwards and backwards while shifted into Park with the electronic parking brake not engaged. The subject vehicle's electronic parking brake logic performed as designed during the inspection and the vehicle experienced no unintended movement while the electronic parking brake was engaged. In its November 21, 2023 response to ODI's information request, Kia provided customer complaint and warranty claim data for 19 unique VINs which relate to the alleged defect in the subject vehicles. Kia separately made ODI aware of one additional allegation of vehicle rollaway that resulted in a collision with another vehicle. This collision was reported to Kia following their cutoff date for the information request response. Kia stated that based on its investigation, the subject vehicles' parking mechanism can be damaged if the gear shift lever is shifted to Park prior to coming to a complete stop. If the parking mechanism is damaged and the electronic parking brake is not engaged, unintended vehicle movement while in Park may occur. However, based on ODI's analysis of the available information, it is apparent that not all allegations of the subject defect which have been investigated by Kia have had evidence that the parking mechanism was damaged. At this time, the root cause of the alleged defect is not yet confirmed. On October 19, 2023 Kia filed safety recall 23V-724 for MY 2015-2019 Kia Soul EV vehicles. The new recall remedy provides update electronic parking brake logic that automatically activates the electronic parking brake when the gear shift lever is in the Park position, regardless of whether the vehicle is powered on or off. At this time, NHTSA has not received any reports of electronic parking brake failure for the subject vehicles, but will continue to monitor following this investigation. In view of the recall action being taken by Kia, ODI is closing this Recall Query. The agency reserves the right to take additional action if warranted by future circumstances. On August 17, 2023, the Office of Defects Investigations (ODI) opened RQ23-004 to investigate instances of loss of motor power due to overheating high voltage battery contactors in model year (MY) 2021-2022 Ford Mustang Mach-E vehicles (subject vehicles). This Recall Query (RQ) was opened after receiving 12 consumer complaints alleging a high voltage battery main contactor failure in subject vehicles that were included in Recall 22V-412 and remedied prior to the reported incidents. The remedy in 22V-412 was a Secondary On-Board Diagnostic Control Module (SOBDMC) software update to monitor contactor temperature and reduce battery power to prevent damage to the contactor, and a Battery Energy Control Module (BECM) software update to monitor contactor resistance to identify an overheated contactor and reduce vehicle power to prevent further damage. On October 13, 2023, Ford filed Recall 23V-687 of 34,762 2021-2022 Ford Mustang Mach-E vehicles produced from May 27, 2020 to May 24, 2022 with recalled Bussed Electrical Center parts NK48-10C666-AA, NK48-10C666-BA, or LK98-10C666-AB. Affected vehicles are equipped with the extended range battery. Ford stated that preexisting damage to the contactors caused by heat and accumulated wear may reduce the effectiveness of the software update remedy in 22V-412. The remedy of 23V-687 is to complete a replacement of the Bussed Electrical Center, or High Voltage Battery Junction Box. In addition, Ford stated that subject vehicles equipped with standard range batteries, in comparison to vehicles equipped with extended range batteries, are less at risk of experiencing this failure due to their smaller battery capacity and lower output electric drive motors. In view of the recall action taken by Ford, ODI is closing this investigation. ODI will continue to monitor reports of this failure in both extended range and standard range models. The Agency reserves the right to take additional action if warranted by future circumstances.
