On September 6, 2007 Keystone RV Company notified NHTSA (07V-411) that certain Irvine Shade & Door glass shower doors with towel bars could break if pressure is applied to the towel bar.The risk of glass breakage can result in personal injury.Keystone
stated that vehicles produced between October 19, 2006 and July 26, 2007 were affected.On December 4, 2008 Coachmen RV notified NHTSA (08V-639) that Coachmen vehicles produced between June 15, 2001 and August 6, 2007 also contained this equipment.On 12/30/2008, RMD mailed Irvine an IR letter requesting its customer list and determination on whether an equipment Defect Report would be filed with NHTSA.After no response, another IR letter was mailed to Irvine on 3/5/2009.Irvine submitted a customer list to NHTSA's Chief Counsel on 6/4/2009 requesting confidentiality for its customer list.However, no letter was submitted answering RMD's questions on whether a Defect Report would be filed.On 11/13/2009, Chief Counsel granted confidentiality to Irvine's customer list while allowing limited release of OEM names for needed RMD mailings.On 12/23/2009, RMD mailed IR letters to 161 companies identified as vehicle manufacturers & new vehicle alterers (out of 502 customers on Irvine's customer list).IR response statistics below: IR letters mailed: 161 IR Responses :72 No response :63 Returned letters:26Of those responding to RMD's IR letter, 75% (54 of 72 companies) stated they had never purchased the subject glass shower doors from Irvine.Irvine supplied many of these companies with a letter stating that the inclusion of their company name on the customer list submitted to NHTSA was done in error.No revised or corrected customer list was ever submitted to RMD.Those companies that did purchase the subject shower doors chose not to file any subsequent Defect Reports citing ongoing reviews of warranty claims or no substantial warranty history at all.On 3/22/2010, another IR letter was mailed to Irvine requesting a corrected customer list and defect determination.No response was ever received.Preliminary referral to NHTSA Chief Counsel indicates this matter will not be addressed immediately due to current workload.Future referral to NCC for civil penalties is likely considering Irvine's inability to respond to several NHTSA information requests under 49 USC 30166.This EQ is closed.

To date, ODI is aware of 142 non-duplicative complaints related to the alleged defect.In addition, ODI is aware of 2,626 subject vehicles with one or more warranty claims related to the alleged defect.In total, ODI is aware of 2,714 unique subject vehicles that have experienced one or more front suspension coil spring fractures.ODI is not aware of any crashes or injuries related to the alleged defect.The engineering design and testing data supplied by GM, along with the frequency of consumer complaints and warranty claims related to the alleged defect, tends to support the position that, in general, the coil springs that fractured on many subject vehicles did not achieve the expected service life.However, in regard to the subject vehicles, a front suspension coil spring fracture itself has little safety consequence, as evidenced by the minimal impact on vehicle ride and handling and by the number of drivers who were unaware of the failure condition for some time until alerted by service personnel.The ratio of tire punctures to coil spring fractures appears low and there are no reports of crashes, deaths or injuries related to the alleged defect. A safety-related defect has not been identified at this time and further use of agency resources does not appear to be warranted.Accordingly, this investigation is closed.The closing of this investigation does not constitute a finding by NHTSA that a safety-related defect does not exist.The agency will continue to monitor this issue and will take further action if warranted by the circumstances.See the EA08-026 Engineering Analysis Closing Report for more details.

ODI opened this investigation based on a review of Early Warning Field reports which indicated instances of the air compressor discharge line coming loose or separating on certain model year 2008 Mack Trucks.Further review indicated that Mack
had issued a Service bulletin (#SB 531014) in May 2008.A revised air line was incorporated into production in May 2008 and the ferrule type fitting was changed to a 37 degree flare compression type fitting.The failure resulted in a diminished air supply to the truck's braking system.During the investigation, ODI collected information from Mack
, examined warranty claims, discussed the problem with fleet managers and confirmed that the subject vehicles meet the requirements of FMVSS 121, (Air Brake Systems).This standard includes a requirement for an air pressure gauge, and an audible and visual warning signal. The air system normally operates at 100 -120 psi.If the air discharge line becomes loose, the system will continue to hold air but replenishment may be diminished depending on the degree of leakage.If the air line separates, air system replenishment will cease and air pressure will drop as system air is used during brake applications.As the air pressure drops, the dashboard mounted gauge provides a continuous pressure status. Drivers are required to have a CDL (commercial drivers' license) with air brake endorsement and part of the training to obtain such a license includes learning how to monitor and respond to the air pressure system gauges.As the pressure drops below 60 psi, the audible/ visual warning device activates alerting the driver to take appropriate action to stop the truck in a safe location.If the air pressure is allowed to drop below 45 psi, the parking brakes will began to apply.ODI confirmed that approximately 94% percent of the trucks have engine brakes that provide supplemental braking and allow a driver with a leaking discharge line to use the compression of his engine to slow the truck, thus further preserving available air pressure.Using warranty claim data, ODI contacted 47 owners/drivers to gain an understanding of the problem and learned that 16 of the 47 discovered the problem while the truck was parked.Of the 31 drivers that discovered the problem while driving only 13 found it necessary to stop on the roadway's shoulder.Of those 13, four made temporary repairs in place and continued driving the truck. The remaining 18 were able to safely drive to a location away from the roadway to wait for assistance.Thus, 38 of the 47 vehicles surveyed were repaired in a garage or facility. Nine of the 47 were apparently towed.ODI has carefully reviewed the warranty claims and concluded that 639 reports are applicable in that warranty was performed on those trucks to replace or repair the defective air compressor fitting.Based on fleet interviews and warranty claim review, ODI believes the vast majority of the warranty claim repairs were performed in safe locations.The data indicates that the majority of failures were resolved off the highway in a safe location due to the warning system required by FMVSS 121 or during the driver's daily truck inspection required by the Federal Motor Carrier
Safety Administration.During the investigation, ODI established that no crashes, injuries or fatalities have occurred as a result of this problemOn May 21, 2009, Mack
