On July 28, 2023, The Office of Defects Investigation (ODI) opened a Preliminary Evaluation (PE23-014) after receiving 12 Vehicle Owner Questionnaires (VOQs) reporting loss of steering control in model years (MY) 2023 Tesla Model 3 and Y vehicles. During PE23-014, ODI interviewed complainants, sent an Information Request (IR) letter to Tesla
, and convened technical meetings with Tesla
that included a joint inspection of a crashed vehicle. The investigation identified 2,388 total complaints regarding the alleged defect, including the 124 complaints identified through VOQs and 2,264 complaints through manufacturer data. ODI is associating 1 crash to this investigation (11544655), where a driver was unable to complete a right hand turn in the intersection and hit a vehicle. This vehicle was subject to a joint inspection by ODI and Tesla
. The steering rack was recovered by ODI with the consumer's permission. ODI's interviews of allegations supplied to the agency, in the form of VOQs, identified patterns of the incoming complaint traffic. Complaints allege an inability to turn the steering wheel, while other complaints allege an increase in required effort to turn the steering wheel. These conditions were reported to occur during the drive cycle as well as at start-up. Approximately one-third of complaints with a reported speed occurred at vehicle start-up or speeds less than 5 mph. Interviews with failures reported at speeds over 5 mph, half reported an inability to turn the steering wheel. A majority of complaints with a reported speed reported between 5 mph and 35 mph with approximately 1 in 10 occurring on open roads at speeds 35 mph and over. The highest reported speed regarding an alleged inability to turn the steering wheel is 75 mph. A majority of allegations reported seeing a warning message, "Steering assist reduced", either before, during or after the loss of steering control. A portion of drivers described their steering begin to feel "notchy" or "clicky" either prior to or just after the incident. There have been multiple allegations of drivers blocking intersections and/or roadways. ODI is aware of over 50 vehicles which were allegedly towed as a result of the condition from a variety of areas including, driveways, parking lots, side of road and intersections. Multiple complaints state they were able to temporarily remedy the condition by power cycling the vehicle, but the issue reoccurred until the steering rack was replaced. Tesla
supplied four steering rack part numbers on MY 2023 Model 3 and Model Y vehicles. PE23-014 identified part numbers, 1044832-00-A and 1188832-00-A accounted for 2,187 of the complaints and 4 did not have original part number in the information request. This Engineering Analysis has been opened to characterize the conditions leading to- and stemming from- failures with steering rack part numbers 1044832-00-A , 1188832-00-A. On January 31, 2024, the Office of Defects Investigation (ODI) opened PE24002 to investigate allegations of loss of motive power in model year (MY) 2021 Ford Bronco Sport vehicles. At that time, ODI had received 31 Vehicle Owner Questionnaires (VOQs), with consumers commonly reporting a loss of engine power accompanied by complete electrical system failure which occurred either while driving, or after coming to a stop. The investigation revealed that MY 2021-2024 Ford
Bronco Sport and certain MY 2022-2023 Ford
Maverick vehicles are equipped with a 12V enhanced flooded battery (EFB), which may experience an abrupt failure during a drive cycle. The failure is the result of an internal corrosion issue, which is influenced by long exposure to high under-hood temperatures. If the sudden loss of 12V battery voltage is not detected by the vehicle's battery management sensor, the vehicle can experience a loss of power event without providing any warning to the driver. Following a loss of power due to battery failure, the vehicle may be without all 12V accessories including hazard lights and typically will not be able to be restarted. In its March 2024 response to ODI's information request, Ford
provided 367 customer complaints (including field reports, lawsuits, and legal claims) and 1005 warranty claims relating to the alleged defect. When combining ODI and manufacturer failure report data, there are 1,368 unique VINs represented across MY 2021-2023 Ford
Bronco Sport vehicles. One of the VOQs submitted to NHTSA reported a minor rear-end collision resulting from a loss of power event. Included within Ford's complaints were a reported single-vehicle collision and a vehicle fire which occurred following a loss of power event at a traffic light (fire cause and origin was not determined). Additionally, across ODI and manufacturer data, there were 12 allegations of either acid leaking from the 12V battery or smoke emitting from the battery area within the engine compartment. On April 12, 2024, Ford
submitted recall 24V267 for MY 2021-2024 Ford
Bronco Sport and certain MY 2022-2023 Ford
Maverick vehicles (total population 456,565). The recall remedy provides updated calibrations to the body control module and powertrain control module. Ford
indicated that the update will result in improved detection of 12V battery state of charge during drive cycles and will provide notification to the driver if the battery significantly degrades while driving. Additionally, Ford
has replaced the EFB with an absorbed glass mat (AGM) battery as both a service component for the subject vehicles and as original equipment for Bronco Sport vehicles with production dates beginning in mid-March 2024. The warranty claim rate for peer vehicles equipped with an AGM battery is substantially lower than that for the subject vehicle population. In view of the recall action being taken by Ford

Electric Vehicles (PHEV) produced between August 12, 2016 through August 7, 2018. The recall addresses ignition "OFF' fires that originate at the high voltage (HV) battery pack
assembly. An ignition 'OFF' vehicle fire can result in an increased risk of occupant injury, injury to persons outside the vehicle, and property damage, with little to no warning. At the time of the recall filing the root cause of the vehicle fires was unknown and under investigation by FCA
and suppliers. A software remedy designed to improve thermal event detectability by monitoring battery diagnostics is currently being installed on subject vehicles as an interim remedy. As of November 23, 2023, the root cause of the battery failure is unknown. From December 2022 through December 2023, NHTSA has received four consumer complaints (VOQs) alleging vehicle thermal events involving recalled 2017 and 2018 Chrysler
Pacifica PHEVs. Additional information collected from complainants confirmed a thermal event originating near or at the HV battery pack
after the Recall 22V077 remedy had been applied to the vehicle. Follow up meetings with FCA
and suppliers in November and December 2023 to discuss the ongoing root cause investigation and post-recall remedy incidents indicated a recent increase in HV battery
thermal events. Furthermore, a review of NHTSA complaint data indicated the post-recall HV battery
thermal event complaint rate now exceeds pre-recall levels. The Office of Defects Investigation (ODI) is opening this Recall Query (RQ) to review the effectiveness of the original recall remedy, understand the root cause of the battery fires, investigate additional reports of Pacifica PHEV HV battery

On January 11, 2024, the Office of Defects Investigation (ODI) opened PE24001 to investigate instances of loss of braking performance in model year (MY) 2023 Fisker Ocean vehicles manufactured by Fisker
Group Inc. The complaints allege an intermittent partial loss of regenerative braking over low traction surfaces, without alerting the driver. ODI is aware of six minor crashes and one injury due to this failure. The subject vehicles utilize a brake blending feature to achieve deceleration. Upon application of the brake pedal, both the service brake system (friction braking) and regenerative braking system (electric motor negative torque) are applied. Regenerative braking on low friction road surfaces may result in wheel slippage due to the high motor negative torque. The vehicle responds by reducing and/or eliminating regenerative braking torque to reduce wheel slip on drive wheels to maintain vehicle stability and steerability. This reduction in regenerative braking may cause a sensation of loss of braking force (or even vehicle accelerating) that was reported by consumers. Additionally, the driver may experience an unusual/altered brake pedal feel during loss of regenerative braking events, requiring them to press the brake pedal harder to maintain the expected braking rate. Upon exiting the low friction road surface, the regenerative braking force is restored. The initial calibration of the regenerative braking system contained aggressive interventions with a slower recovery in the event of loss of regenerative braking. In November 2023, Fisker
launched an Over-the-Air update (OS1.10 SW). This update included a recalibration to reduce regenerative braking torque less aggressively and allow torque recovery to occur more quickly. Based on consumer reports and the data that Fisker
provided in response to ODI's inquiry, OS 1.10 SW reduced, but did not eliminate, the sensation of loss of braking force and resulted in stopping distance performance improvements. Friction braking is always available when the driver presses the brake pedal, and its performance is unaffected by regenerative brake loss. Fisker's braking system is designed to reach full deceleration even with an unavailable regenerative braking system. The partial loss of braking force during moderate braking events can be overcome by applying additional pressure to the brake pedal to slow or stop the vehicle. On August 20, 2024, Fisker
filed recall 24V-623 for all 2023 MY Fisker
Ocean vehicles produced for the U.S.A. The recall remedy includes OS 2.2 over-the-air update, which is expected to modify the brake module to enhance the vehicle brake system's ability to adapt to various driving scenarios. With successful implementation of OS 2.2, a road disturbance will not trigger a reduction in regenerative braking torque. In view of the recall action being taken by Fisker

Summary
The Office of Defects Investigation (ODI) has received 12 complaints and multiple field reports alleging incidents of stall/loss of motive power as a result of low-pressure fuel pump failures in certain 2017-2018 model year (MY) Alfa Romeo Giulia equipped with 4 cylinder 2.0L and 6 cylinder 2.9L turbocharged engines. Based on an analysis of the complaints and field reports, ODI has found that most of the reported loss of motive power incident allegations occurred at speeds above 25 MPH and resulted in permanent disablement of the vehicle. One (1) complaint reported a minor crash incident attributed to the loss of motive power due to fuel pump failure. Other complaints reported the fuel pump failed, was repaired and the fuel pump failed a second time. During discussions with the manufacturer, ODI learned that in January 2018, they released a Technical Service Bulletin (TSB) #18-001-18 announcing to its service dealers a remedy for vehicles who experiences a diagnostic trouble code (DTC) P008A-Low Pressure Fuel System Pressure- Too Low. The remedy includes inspection of the fuel pump module and replacement, if necessary. A Preliminary Evaluation has been opened to assess the scope, frequency, root cause(s) and consequences of these loss/stall of motive power incidents. Additionally, ODI may expand the scope of the subject population during the investigation, if needed.
