To date, ODI has interviewed several complainants who confirmed that the headlights fully extinguish, and remain extinguished, on an intermittent basis.In most cases this occurred while the vehicle was in motion.The occurrences do not show a repeatable pattern, and the problem occurs at a low frequency, typically one or more weeks between failures.For this reason most complainants have not attempted diagnosis or repair of the problem, so the cause is unknown, and the problem continues to occur.In the cases where repairs have been attempted, which consisted mainly of headlight switch and body control module replacements, mixed success is reported.In interviews complainants advised ODI that they can usually toggle the headlight switch, or turn on the high beams by actuating the column stalk (pulling it upward), when the headlights unexpectedly turn off.Complainants report that toggling the switch is usually effective, and the high beams have always been available when the column is used.During the course of the interviews ODI was able to obtain a few suspect headlamp switches for further evaluation.No verifiable crashes or injuries have been identified in the vehicle owner complaints (VOQs).Chrysler notes that its complaints also include allegations of dimming or flickering of the headlights in addition to loss of headlamps, and that their analysis of available data indicates a declining trend occurring in the field.Chrysler
data reports two alleged crash incidents; one where a consumers vehicle lost forward lighting and struck a deer, and one where a consumer alleged their lighting stopped working, causing them to depart the roadway and drive into a ditch.A minor injury is alleged in this second incident.Both incidents occurred several years ago, and neither allegation has been confirmed by ODI.ODI is upgrading this investigation to an Engineering Analysis (EA11-010) to continue its analysis of the alleged defect and its consequences, which will include additional analysis of consumer reports, their frequency and trend, and Chrysler
's technical information for the headlight system and its components.Additionally testing will be conducted to evaluate possible root cause(s) of the alleged defect, and will include analysis of suspect components collected from the field, and/or assessment of subject vehicles that have reportedly experienced the condition.

Complaints allege that while refueling the vehicle, fuel can spill out or spit back from the fuel filler port on model year (MY) 2003 Kia Sorento vehicles (subject vehicles).Consumers report that the spilled fuel can splash on the person fueling the vehicle, or on the side of the vehicle and the ground.The subject vehicles are equipped with an evaporative emissions control system that, among other things, manages air exchange between the fuel tank and the atmosphere in an environmentally safe manner.The system consists of a vapor canister, various control valves and tubing, and a canister filter (fuel tank air filter) through which the air that flows in and out of the fuel tank must pass.Kia
's testing found that if the canister filter becomes significantly clogged, excess pressure can build up in the fuel tank and cause fuel to spit back from the filler port during refueling.Kia
has stated that the canister filter requires regularly scheduled service maintenance; however, the component is not listed as a maintenance item in the owner's or service manual for the subject vehicles.ODI notes that Kia
does list the filter on the maintenance schedule in the owner's manual for MY 2005 and newer Kia
Sorento vehicles.To address the issue of fuel spit back during refueling, Kia
will conduct a service campaign of approximately 86,000 MY 2003 and 2004 Kia
Sorento vehicles.The campaign will provide a one-time replacement of the canister filter, at no cost to the owner.Kia
will begin notifying the affected owners in late January 2012 and will inform the owners that the filter must be serviced at regular intervals.Kia
will also provide an updated service maintenance schedule.Consumers should refer to the service campaign bulletin number SC092 available in the investigative file.A safety-related defect trend has not been identified at this time and further use of agency resources does not appear to be warranted.Accordingly, this investigation is closed.The closing of this investigation does not constitute a finding by NHTSA that no safety-related defect exists.The agency will monitor the issue and reserves the right to take future action if warranted by the circumstances.

The Dorel Juvenile Group (DJG) child seats that are the subject of this investigation are equipped with a center front adjuster (CFA) and were manufactured in 2008 and 2009. The CFA functions as a locking mechanism that locks the shoulder harness in an adjusted position.After the harness of the child seat
has been placed over the child's shoulders and buckled, it must be adjusted, or tightened, by pulling out on the tightening strap running through the CFA.The CFA is spring loaded to a normally closed position and the ?locking pawl (pawl) locks the harness tightening strap of the CFA in the adjusted position.The pawl permits movement of the strap in the tightening direction only.In order to release or loosen the harness adjustment, the pawl must be pressed down to open the CFA and unlock the adjusting strap.DJG's CFA is not used by other child seat
manufacturers.The Office of Defects Investigation (ODI) opened this investigation as PE10-009 on March 5, 2010 based on two reports from a consumer alleging the CFA would not securely lock the harness on two different model year DJG Vantage Point child seats
.Based on information provided by DJG and additional consumer reports received by ODI on other model DJG child seats
that use the CFA, the investigation was upgraded to EA10-005 on July 2, 2010.The scope of the investigation was expanded to include all DJG child seats
manufactured in model years 2008 and 2009 equipped with the CFA.The consumer reports associated with the investigation allege the occupant harness slips while in use and becomes loose.A loose harness increases the risk of injury in a crash.There were no reports of CFA failure in a crash.On February 10, 2011, DJG submitted a Defect and Noncompliance Information Report to conduct a safety recall (11C-001) of certain subject child seats
to remedy a condition where the pawl does not fully engage the harness tightening strap in the CFA.The recall affects approximately 795,000 seats manufactured from May 1, 2008 through April 30, 2009 and includes the following models:Prospect, OnBoard, Alpha Omega Elite, Alpha Omega, Mico, Enspira, Priori, Vantage Point, Avenue and Uptown.Seats of these models that were manufactured prior to May 1, 2008 and after April 30, 2009 are not included in the recall because the CFA was produced using a different process. ODI continued its investigation (EA10-005) as there were other models with a CFA manufactured from May 1, 2008 through April 30, 2009 that were not included in the recall scope.These models include the Summit, Scenera, Hi Back Booster, Designer 22, Apex, and Touriva.As part of this investigation, NHTSA's Vehicle Research and Test Center (VRTC) conducted extensive evaluation and testing, including FMVSS 213 sled tests, on both recalled and non-recalled models.The investigation did not reveal the existence of a safety-related defect trend in the CFA in the models that were not recalled.
