The Office of Defects Investigation (ODI) has identified 128 Vehicle Owner Questionnaire (VOQ) reports alleging the Active Head Rest (AHR) inadvertently deployed without the occurrence of a crash or other rear impact event in model year (MY) 2014 Jeep Grand Cherokee and Dodge
Durango, the subject vehicles, produced by Fiat
Chrysler
Automobiles (FCA
). Thirteen of those reports allege injuries mostly involving headaches and soft tissue injuries, however some reports allegedly resulted in disorientation, nausea and vomiting according to complainants interviewed. A small number of EWR field reports have also been identified.The AHR, which is intended to deploy in rear impact crashes to reduce the risk of neck injury, is a spring actuated device controlled by the air bag control unit (ACU).When the ACU senses a rear impact, it signals the AHR to deploy (forward from the headrest, and towards the occupant's head) to limit rearward head motion in the crash. In the reported cases, it appears a plastic material inside the AHR, intended to hold the AHR in the "ready" (compressed) state, fails due to stress and/or fatigue allowing the AHR to actuate unexpectedly. Inadvertent deployments have reportedly occurred at either (or both) front seat positions, when the vehicle is either occupied or unoccupied, when the seat is either occupied or unoccupied, and in some cases (nearly two-thirds) while the vehicle was in motion. The AHR must be replaced to correct the condition, a process that apparently involves disassembly of the seat.ODI is opening this preliminary evaluation (PE) to evaluate the scope, frequency and consequence of the alleged defect in the subject vehicles. The Office of Defects Investigation (ODI) has completed its review of a defect petition received on 14 August 2019 requesting an investigation of an alleged defect of the manual transmissions in Model Year (MY) 2019 Volkswagen (VW
) GTI, Golf GTI and Jetta GLI vehicles. The stalling incidents cited occurred without warning as the vehicle slows to a stop or prepares to make a turn across traffic with the clutch pressed in and the vehicle in neutral.The hazard posed by a vehicle stalling event is manifested in the inability of the vehicle to move with the flow of surrounding traffic. Two factors have a major impact on the potential hazard to the vehicle, its occupants and surrounding vehicles: the surrounding traffic speed and the subject vehicle's restart ability. In the case of the vehicles that are the subject of this petition, restart is immediate and the stalls occur when the vehicle is slowing to stop for a traffic signal, making a turn, or is completely stopped. These circumstances substantially reduce the risk of harm to the vehicle, its occupants and surrounding vehicles.As of March 2, 2020, out of the population of 11,333 subject vehicles, NHTSA has identified 214 consumer complaints in its database, with unique Vehicle Identification Numbers (VIN), citing engine stalling as vehicles are being brought to a stop and/or preparing to make a cross traffic turn. When combined with the warranty, field report and customer complaint data received from VW
in response to the Information Request letter sent, NHTSA identified 413 unique VINs alleging low speed engine stalling.Volkswagen
determined that the low speed / idle engine stall was the result of unwanted gases remaining in the cylinder caused by the intake valve camshaft not being in the correct position at idle. The intake camshaft is in an advanced position. This advanced position results in too much overlap with the position of the exhaust valve camshaft, allowing both the intake and exhaust valves to be open at the same time. The erroneous advanced position of the intake camshaft is a result of low oil pressure at idle and high oil temperatures, greater than or equal to 110?C/230?F combined with engine control algorithms in the Engine Control Module (ECM) that position the intake camshaft. Based on examination of returned engines, only engines manufactured at the Silao Mexico plant were affected due to a tolerance stack-up issue with the oil system of the engines.In December of 2019, VW
initiated a Service Action (24FD) to remedy the stalling issue in the affected vehicles. The warranty for the affected vehicles was extended to December 31, 2025. The software in the Engine Control Module (ECM) will be updated with a new calibration value for the adaptation nodes of the regulation valve. This is a directed action with letters being sent to vehicle owners asking them to bring their vehicles to their dealership to have the software update installed in their vehicle. The letters were sent in December 2019.After thoroughly assessing the material submitted by the petitioner, information already in NHTSA's possession, information submitted by Volkswagen
in response to an information request and the potential risks to safety implicated by the petitioner's allegation, NHTSA does not believe that the stalling condition as alleged by the petitioner indicates the likelihood of a safety related defect that would warrant a formal investigation, therefore the petition is denied. A defect petition (see DP19-002) was received July 23, 2019 alleging that the occupant classification system (OCS) sensor (mat) in model year (MY) 2011 Nissan Leaf vehicles is defective and affects the proper deployment of the passenger front air bag. The petitioner provided a list of potentially related NHTSA reports and noted a Nissan
recall involving later MY Leaf vehicles with OCS defect. NHTSA is granting the petition and opening a preliminary evaluation in response, please see PE19-015.Since receiving the defect petition, the Office of Defects Investigation (ODI) has determined that the MY 2011 and 2012 Leaf share a common OCS design, both using the same sensor mat for passenger presence sensing. Starting in MY 2013, Leaf OCS was changed to a design using a different sensing system, one that did not use the mat present in prior model year, Nissan
Leaf. While MY 2013-2016 Leaf vehicles were recalled for an OCS defect under recall 16V-244, a recall influenced by ODI investigation EA15-004, earlier MY Leaf vehicles were not included because they were not affected by the particular defect involved in that recall.ODI identified and reviewed 13 vehicle owner questionnaire (VOQ) reports in its database, one of which is a duplicate, involving MY 2011 and 2012 Leaf vehicles that allege a problem with the OCS. The VOQs indicate that an OCS warning light illuminated at which point the vehicle was diagnosed and it was determined the sensor mat was the cause of the OCS problem. Most of the VOQs also noted the high cost of repair, which apparently involves replacement of the passenger front seat, according to complainants. None of the VOQs allege a problem with the performance of the passenger air bag, an injury, or a fatality due to the failure of the OCS.During the evaluation of this petition, ODI did not request information from Nissan
regarding the complaint data in its possession, and accordingly the manufacturer complaint counts shown above are marked as not applicable. The Office of Defects Investigation (ODI) opened PE19-009 to investigate 51 complaints and additional Early Warning Report (EWR) data related to allegations of front passenger seat Occupant Detection System (ODS) malfunctions in model year (MY) 2016 through 2018 Subaru Forester vehicles. These vehicles use a capacitive type ODS, which is intended to classify the occupant of the passenger seat as either full-sized, small stature, a child seat
or other object. The complaints reported the ODS failed to operate properly, the SRS (Supplemental Restraint System) warning light illuminated, and/or the ODS incorrectly suppressed (turned off) the front passenger air bag when the seat was occupied by a full-sized passenger. In a frontal crash that is sufficient to warrant deployment, a suppressed air bag may increase the risk of injury to the occupant of the seat.On August 14, 2019 ODI sent an Information Request (IR) to Subaru
requesting information on the subject vehicles and MY 2014-2015 Subaru
Forester vehicles.On October 3, 2019, Subaru
submitted a Defect Information Report (DIR) to NHTSA describing a safety defect in the passenger-side front airbag Occupant Detection System (ODS) harness of 366,282 MY 2015-2018 Subaru
Forester vehicles equipped with seat heaters.The DIR stated that over time, due to a decrease in contact pressure between the terminals in the ODS sensor mat harness, a temporarily unstable electrical connection may be possible. If the connection is temporarily unstable, the ODS may not properly determine the status of the right front seat occupant. If the ODS is unable to correctly determine the status of the front occupant, the airbag warning light may illuminate, and the front passenger airbag may deactivate even though the seat is occupied, increasing the risk of injury to the front passenger in the event of a crash.Using vehicle production data and field occurrence frequency, Subaru
determined the potentially affected vehicle production range to be between January 20, 2015 ?and August 1, 2017.In addition, Subaru
indicated that over 99 percent of the related MY 2015-2018 field reports with vehicles in the production range involved vehicles with seat heaters.NHTSA's analysis of complaint and warranty claim data showed 1,440 of the complaints and 11,672 of the claims involve vehicles with seat heaters that were built within the production range covered by Subaru
's recall.In addition, NHTSA's analysis showed a complaint rate of 393.1 per hundred thousand vehicles for the recalled population and 89.0 for MY 2014 through 2018 vehicles that are not included in the recall. Analysis of warranty claim data found a claim rate of 3.2 percent for the recalled population and 0.3 percent for MY 2014 through 2018 vehicles that are not included in the recall.Subaru
will instruct owners with Subaru
Forester vehicles produced in the affected vehicle production range, to bring their vehicles to a dealer to have the sensor mat harness, inspected and replaced with a new part, including the seat cushion assembly, if necessary.Based on ODI's analysis, the actions taken by Subaru
mitigate any unreasonable risk to safety posed by ODS sensor mat failures in the subject vehicles. For subject and peer vehicles outside the recall population and those equipped without seat heaters, ODI's analysis indicates that there is currently no safety-related defect trend. Given these circumstances, further use of the agency resources in this matter does not appear to be warranted. Accordingly, this investigation is closed.

and ODI failure reports are from vehicles registered in so called "salt belt" states. No crashes, injuries or fatalities have been confirmed as of this date.Nissan
's IR response indicated the MY 2013 through early 2018 Nissan
Altima shared a common rear lower control arm design. Accordingly ODI has expanded the scope of the investigation to include all potentially affected vehicles. Nissan
reported implementing a design change in January 2018 to improve the durability of the lower control arm, and also provided an analysis indicating that the design change resulted in a significant (which Nissan
estimated between 5 to 9 times) improvement to its durability.Nissan
acknowledges that a crack may develop in the lower control arm due to stress and loading from normal use of the vehicle, and that road salts commonly used for snow and ice treatment results in corrosion that exacerbates growth and progression of the crack. Once the crack develops, continued use of the vehicle causes the control arm to separate at one of four attachment points that connect it to the vehicle chassis. Nissan
determined the current incident rate was 0.003%. Based on testing it conducted as well as observations from field failures, Nissan
maintains the separation, whether it occurs either statically or while in motion, is readily detectable, that the separated component does not contact the road surface or braking components, and that the vehicle remains controllable and can be safely brought to a stop. Nissan
states that due to low incident rate, high detectability, and low risk of adverse vehicle dynamics, it does not believe the subject control arm failure poses an unreasonable risk to motor vehicle safety.Nonetheless, Nissan
has indicated its intentions to conduct a non-safety, proactive field action for MY 2013 Altima in specific states with high salt usage. Launch of the potential action would be in phases, likely beginning in late 2019 in salt belt states with higher incident rates, and would involve replacement of the lower control arms. Components recovered from the field action are expected to be used to study additional remedy options.During the Engineering Analysis, ODI will continue to collect and analyze complaint and field data in its efforts to fully assess the scope and frequency of the alleged defect. More importantly, ODI will also seek additional information and data regarding the potential safety consequences of rear lower control arm failure, both from evidence collected from field failures and through more rigorous and comprehensive vehicle testing to be conducted by either Nissan
