The Office of Defects Investigation (ODI) opened Preliminary Evaluation (PE21-014) after receiving four complaints from two consumers and identifying Early Warning Reporting (EWR) Death & Injury (D&I) claims alleging injuries involving production year 2018 Cybex Sirona M car (child) seats. According to the reports, child occupants were able to access a foam material, used in the seat headrest, via an opening in its covering material. When doing so, the child subsequently places foam in their mouths thereby resulting in coughing, gagging, vomiting, or passed by means of bowel movements. No severe injuries are alleged in any of these reports. On August 9, 2021 ODI sent an Information Request (IR) letter to Cybex Sirona M distributor Columbus Trading-Partners (CTP) USA Inc. requesting information related to the subject seat. On September 20, 2021, CTP responded with the requested data and provided their position that the concern did not represent a substantial hazard to children. CTP stated that although they were aware of children having picked small pieces of foam from the subject child seat and ingesting it, they were not aware of any claims of serious injury or death. CTP also stated that after learning of the claims of children having access to and picking off pieces of headrest foam, they made a change in production on August 31, 2018 that would prevent the problem from occurring. CTP claims they have not received any complaints on seats manufactured after that date.Following a review of the IR response data and related precedent recalls, ODI conducted multiple meetings with CTP to explain the agency concerns. With this information CTP decided to submit a voluntary safety recall to remedy the subject child seats manufactured prior to August 31, 2018.On April 20, 2022 recall 22C-001 posted recalling certain Cybex Sirona M Convertible Child Car Seat, model numbers 518000385, 518002153, 518000387, 518002145, 518002149, 518002151, and 519000211, manufactured prior to August 31, 2018.With recall action 22C-001 taken by CTP, this investigation is closed as further use of agency resources does not appear to be warranted.The ODI reports cited above can be reviewed at: http://www-odi.nhtsa.dot.gov/owners/SearchNHTSAID using the following complaint identification numbers: 11113133, 11150030, 11150031, 11150032

The National Highway Traffic Safety Administration (NHTSA) Office of Defects Investigation (ODI) received a manufacturer communication, sent to subject 2020 UPPAbaby Knox Convertible Child seat owners, that alerts them the seat can exhibit lateral movement when used in rear facing position.The lateral movement can occur despite secure installation. The communication goes on to instruct owners to stop using the subject car seat immediately and that if they continue to use the seat it will be at their own risk. No fatalities have been reported.The manufacturer produced a reported 1,808 Knox Convertible Child seats and as of late 12/18/2020, indicated 100% of purchasers were contacted but that 411 seats had not been returned. The manufacturer outreach to consumers involved a letter, in which UPPAbaby requested owners to refrain from using the car seat. A second letter provided instructions for owners on how to disable the seat from further use and receive a monetary refund for the seat plus a fifty dollar incentive for doing so.ODI is opening this investigation to evaluate the scope, frequency, and consequence of the alleged defect in the subject 2020 UPPAbaby Knox Convertible Child seat.

NHTSA's Office of Defects Investigation (ODI) has received two complaints and verified a third allegation in a public forum alleging fuel loss from the subject vehicle originating from the fuel fill location while in motion. The Model Year (MY) 2010 ? 2015 Ferrari 458 (subject vehicle) has two built-in systems that are designed to prevent fuel loss: the capless fuel filler and the anti-spit back valve. Damage to the capless fuel filler was noted in all three incidents. Failure of the capless fuel fill neck and/or the anti-spit back valve to seal the fuel system may allow fuel loss from the vehicle during left-turn maneuvers. Fuel loss amounts may vary based on the lateral load exerted during these maneuvers and the level of fuel in the system.ODI is opening this preliminary investigation to characterize the scope, severity, and circumstances of the reported conditions and to fully assess their safety impact. In May 2021, the Office of Defects Investigation (ODI) opened a Preliminary Evaluation (PE) of complaints received alleging a loss of motive power while in motion at any speed with no prior warning to the driver in the 2020-2021 model year (MY) Porsche Taycan electric vehicles. ODI had received 9 Vehicle Owner Questionnaires (VOQs) and several field reports. All complaints alleged that a loss of motive power occurred without warning and error messages about a battery fault displayed during or after the vehicle stalled.In the subject vehicles, a pulse-controlled inverter is mounted on each of the two drive modules on the front and rear axles. The software designed to continuously monitor end-to-end communication between the safety-related electronic control units (ECUs) in the vehicle can, in certain instances, incorrectly and sporadically detect a potentially faulty communication, set an error memory entry, and trigger a shutdown of the power train, resulting in loss of motive power. If this occurs, the affected vehicles display the warning message ?Motor control error. Stop vehicle in a safe place in the instrument cluster.Porsche has corrected the software malfunction in production. Further, Porsche is conducting a safety recall (21V-486) to upgrade the software on vehicles already in the field. In view of the recall action, this Preliminary Evaluation is closed. NHTSA's Office of Defects Investigation (ODI) opened a Preliminary Evaluation after reviewing field reports concerning alternator front bearing failures that cause elevated temperatures within the engine compartment, resulting in the fire suppression system (FSS) activating, immediately shutting the engine down, and rendering the bus inoperable.Analysis of the data provided by Prevost Car (US) Inc. showed high failure frequency and thermal events. An immediate shutdown of the bus can increase the risk of a crash or injury of bus occupants. In response to this investigation, Prevost Car (US) Inc. and Volvo Bus Corporation decided to conduct recalls 22V-221 and 22V-222. By submitting Part 573 Defect Information Reports on April 6, 2022, Prevost Car (US) Inc. and Volvo Bus Corporation notified ODI of a safety defect in certain model year (MY) 2010-2023 Prevost H3-45; X3-45; X3-45 Com; H3-45 VIP: X3-45 VIP and 2020 Volvo 9700 vehicles; some of these buses may have experienced thermal events after failure of the alternator front bearing, because the alternator belt continues to turn the alternator after the bearing failure.These recall actions address the safety concern that led to the opening of this investigation.Therefore, with these recall actions, this investigation is closed. The recalls encompass all model and MY vehicles addressed by the investigation. However, the closing of this investigation does not constitute a finding by NHTSA that a safety-related defect does not exist in other model or MY vehicles outside of the recall scope. The agency reserves the right to take further action if warranted by the circumstances.
