On July 17, 2023, the Office of Defect Investigation (ODI) opened this Recall Query (RQ) after receiving 2 consumer complaints alleging loss of vehicle forward power and/or loss of transmission torque of the rear wheels in model year (MY) 2020-2022 Ford Explorer vehicles (subject vehicles). The RQ was opened to focus on the effectiveness of the remedies of Recalls 22V-255 and 23V-199 related to the alleged problem. Recall 22V-255 involved replacing the bushing and axle cover in the following subject vehicles: 3.3L FHEV Police built before October 14, 2020, and 3.3L Gas Police vehicle built before January 31, 2021. For 2.3L RWD, 3.0L PHEV, 3.3LFHEV, and 3.0LST retail (non-police) vehicles, the recall provided an PCM software update to engage the Electronic Park Brake (EPB) when Park is commanded. The recall addressed a breakage of the rear axle bolt that can cause the driveshaft/half shafts to disconnect, resulting in a loss of transmission torque to the rear wheels necessary to hold park. If the parking brake is not applied, the loss of the primary park torque will allow the vehicle to roll in park increasing the risk of crash and injury. Recall 23V-199 addressed 674 of the subject vehicles produced from February 2, 2019 to April 4, 2022, which received an incorrect remedy under Recall 22V-255. The affected vehicles had received a previous Powertrain Control Module (PCM) update which did not include an automatic EPB engagement when vehicle is shifted to park. On October 17, 2023, Ford
filed a new safety recall (Recall 23V-675) covering 238,364 MY 2020-2022 Explorer vehicles with the following powertrains: 3.3L FHEV Police, 3.3L Gas Police, 3.0L Gas Police, 2.3L RWD, 3.0L PHEV, 3.3L FHEV, 3.0L RWD and 3.0L AWD, produced from October 22, 2018 to July 6, 2022, equipped with a 3-point mounted axle design. stated that the cause of the bolt fracture is due to joint design not robust to peak axle input torques and manufacturing variability. The primary contributor is insufficient bearings for maximum joint loads. This can result in bearing deformation, increasing bending stress on the bolt, which may lead to fatigue failure. stated that the Explorer powerpack variants not covered under Recalled 23V-675do not present an unreasonable risk to motor vehicle safety due to the combination of lower peak torque levels and torque management strategies to share inputs between front and rear axles, reducing occurrence of higher torque. On June 8, 2023, the Office of Defect Investigation (ODI) opened PE23011 to investigate allegations of loss of motor power on model year (MY) 2022 Hyundai Ioniq 5 vehicles. Some consumers reported loss of motor power immediately after a warning lamp illumination or reduction of motive power while the vehicle was in motion. This investigation found that the Integrated Charging Control Unit (ICCU) may be subject to various conditions such as overcurrent, overvoltage induced at the end of charging, and certain thermal loading during operation that can damage its internal components and open the ICCU fuse, resulting in an inability to charge the 12-volt battery. The vehicle could eventually lose all motive power, increasing the risk of a crash. In July 2023, Hyundai

On June 8, 2023, Office of Defects Investigation (ODI) opened Preliminary Evaluation PE23009 to investigate reports of air pressure loss from tires equipped with Danko Manufacturing/RViBrake's (RViBrake) Tire Patrol device, an aftermarket tire pressure monitoring system. ODI's initial inquiry determined that RViBrake was aware of 408 sensors being returned under warranty for leaking pressure, including approximately 40 that were reported to have broken in half, leaving the tires' valve stem core pressed open and leaking air. ODI sent an Information Request (IR) letter to RViBrake on June 8, 2023. By analyzing the IR Response data, ODI learned that the bulk of the sensors returned under warranty were for dead sensors, or minor but consistent air leaks. The manufacturer believes that many of the warranty claims are user-induced, such as damage from curb strikes or from car washes. However, 41 warranty claims were for lost upper sensor halves, a condition that can cause the tire to deflate, without notification to the operator. This condition is particularly concerning when it occurs to one of the vehicle's steer axle tires. No crashes or injuries are known to be directly attributable to any of these sensors. RViBrake explained that in the event of a sensor half detachment, the operator would not immediately be warned. The system refreshes the tire pressure information every three minutes, but it is not uncommon for a packet of information to not be received due to interference, especially with the rear wheels, which may be further away from the receiver. If the system does not receive fresh information from the sensor three times in a row, the icon for that sensor becomes greyed out on the display screen. Furthermore, if the system does not receive information from the sensor five times in a row, an audible alert sounds to warn the operator. As part of the IR response, RViBrake supplied a video replicating the worst case scenario.In the video, a broken and separated sensor half was installed on a fully pressurized steer tire on a Class-A motorhome.The tire deflated by approximately twenty seven percent in ten minutes. With the majority of the air still remaining in the tire after ten minutes, it is believed that the vehicle could still be safely maneuvered off of the roadway, illustrating that a broken Tire Patrol sensor does not lead to an immediate or rapid air loss of the tire. After discussions with RViBrake to discuss ways to reduce the risk to operators, the company has pushed out a system software update that now triggers the audible alert after a loss of signal from the front tires in less than nine minutes. Due to the amount of time needed for a tire to deflate after a full sensor half detachment and in view of this software update action taken by RViBrake, which provides improved notification to the driver of a slow loss of pressure in the tire, ODI has closed this Preliminary Evaluation. The closing of this investigation does not constitute a finding by NHTSA that a safety-related defect does not exist.
