On February 1, 2010, Indiana Mills & Manufacturing, Inc. (IMMI) notified NHTSA that it sold certain school bus seats, manufactured between October 6, 2009 and January 25, 2010, and that these seats did not meet FMVSS No. 222, School bus passenger seating and crash protection, when installed in a school bus application. IMMI stated that certain seat cushion locking mechanisms are capable of being misaligned during vehicle assembly or after the vehicle is in use. A seat cushion with a misaligned locking mechanism might not be able to meet the requirements of FMVSS No. 222 as it would not allow the cushion to remain attached to the seat during a certain threshold of upward force. Should a school bus seat cushion become detached from the seat during vehicle operation, personal injury is possible to the seat occupant(s).On April 2nd, 2010, RMD mailed IR letters requesting 573 Defect Report submissions to 13 of the 14 purchasers IMMI listed in its February 1st letter to the agency.Daimler Trucks
(Thomas Bus) had already filed a Defect Report for its buses in February (10V-058).After a second IR letter was mailed to the other 13 purchasers on June 17, 2010, RMD received a response from all purchasers with the exception of Durham School Services.As a school bus operator and not a vehicle manufacturer, they would not be required to notify the NHTSA and submit a Defect Report, but RMD did want to notify them of this safety issue.Of the 12 respondents, 4 chose to file a subsequent Defect Report for school buses they built with these seats:Collins Bus Corp:10V-291 Starcraft Bus: 10V-194 Bus West: 10V-292 HA Dehart & Sons: 10V-142The remaining 8 responses did not require a Defect Report as the vehicles were repaired prior to sale, the seats were previously returned to IMMI, or they were not installed into buses as original equipment.This EQ is closed.EQ responses can be found in the Document Repository.A working spreadsheet of responses can be found by clicking on the paper clip in this Adobe document.

The Office of Defects Investigation (ODI) received Defect Petition DP10-003 on March 2, 2010, requesting the investigation of model year (MY) 2003 Chevrolet Silverado 2500HD 4WD pickup trucks for corrosion failures of the vehicle brake lines.DP10-003 was granted and on March 30, 2010, Preliminary Evaluation PE10-010 was opened on more than six million model year 1999 through 2003 light trucks and sport utility vehicles manufactured and sold by General Motors Corporation (GM).On July 2, 2010, ODI received GM's response to an information request, which included GM's assessment of the frequency and safety consequences of the alleged defect. GM stated that:(1) the brake system of the subject vehicles is split front/rear and should a brake pipe suddenly fail for any reason, the affected vehicle would be capable of stopping with the pressure supplied by the remaining circuit; (2) the subject vehicles were designed to meet the hydraulic circuit partial failure requirements of Federal Motor Vehicle Safety Standards 105 and 135, Light Vehicle Brake Systems; and (3) should a brake fluid leak occur for any reason, the brake system malfunction indicator lamp (MIL) would illuminate and warn the driver before the brake fluid level was low enough to cause a loss of line pressure.ODI's and GM's analysis indicates that 85 percent of the complaints occurred on vehicles used primarily in States that use salt on the roads during Winter months (Connecticut, Delaware, District of Columbia, Illinois, Indiana, Iowa, Maine, Maryland, Massachusetts, Michigan, Minnesota, Missouri, New Hampshire, New Jersey, New York, Ohio, Pennsylvania, Rhode Island, Vermont, West Virginia and Wisconsin).Overall the complaint rates per 100,000 vehicles for Salt Belt States is 43.0 and Non-Salt Belt is 3.0.In approximately 25 percent of the complaints the brake pipe failure occurred suddenly, with no warning (i.e., no MIL, Brake Fluid Loss Warning), and resulted in extended stopping distances.In 30 of these the increase in stopping distance that resulted was a factor in the crash and in 10 others the vehicle was intentionally steered off the road or into another lane of travel in order to avoid a crash.This investigation has been upgraded to an Engineering Analysis (EA11-001) for subject vehicles sold or currently registered in Salt Belt States to further assess the scope, frequency and safety risks associated with sudden failures of corroded brake pipes that can result in decreased brake effectiveness. ODI will continue to gather information on subject vehicles outside the Salt Belt as well.

On January 27, 2010 the National Highway Traffic Safety Administration (NHTSA) was petitioned by Thomas M. Gisslen, that it conduct an ?investigation of certain products manufactured by Dayton Wheel Concepts, Inc. and that the agency issue an order requiring Dayton Wheel to remedy certain alleged design defects.In assessing the petitioner's claim that the subject Dayton products are defective, NHTSA reviewed all reasonably available information (including Dayton's responses to our information requests) to determine whether the products were failing in real-world use and, if so, how frequently.After conducting a comprehensive effort to uncover reports of Dayton wheel separation and/or collapse or motorcycle drive pulley failure, we found no such reports concerning Dayton wheels and one (from 2007) involving a drive pulley, the latter apparently resulting from improper installation.Based on that review, we found no evidence that a safety-related defect trend, related to the petitioner's allegations, currently exists.Therefore, in view of the need to allocate and prioritize NHTSA's limited resources to best accomplish the agency's safety mission, the petition is denied. However, we will continue screening for related reports and will take further action if warranted.
