ODI became aware of a fire involving a Washington Metro Area Transit Authority (WMATA) bus.A WMATA contracted vehicle inspection identified the failure of a tapped hydraulic fitting to have caused the fire. Orion VI buses were built from 1994 to 2002.This investigation is specific to 1995, 1997-2000 production.Daimler Bus informed ODI that 1994 & 1996 model year buses were either prototypes or sold only in Canada; buses built after 2000 had a tee fitting in lieu of the tapped fitting in question.On October 20, 2010 Daimler Bus informed ODI that an internal meeting was planned to discuss the same concern as the one that prompted this investigation.ODI agreed to delay formally requesting information; an IR letter was to be mailed on November 9, 2010 if needed. On November 2, 2010 ODI received a 573 Defect Report (Recall 10V-538) that addresses the concerns of this investigation.This investigation will be closed.There is no associated IR letter.A copy of the defect report is attached.

On June 22, 2010, Caterpillar, Inc. reported that certain C7 diesel engines, manufactured between March 3, 2005 and January 6, 2009, contain two fuel lines that may develop a crack at the fastening point to the common fuel rail and cause a fuel leak.A fuel leak, in the presence of an ignition source, can result in a vehicle fire.This issue was assigned recall number 10E-025.On October 30, 2010 the Recall Management Division (RMD) mailed two Information Request (IR) letters to both General Motors (GM) and Kenworth Truck Company (Kenworth) as they were the only two manufacturers that had not yet filed subsequent safety recalls for their vehicles.PACCAR, Kenworth's parent company, submitted a response on November 2, 2010 stating their recalled C7 engines were only used in prototype chassis that were not retailed to the public.GM submitted their response to RMD on November 5, 2010 claiming they did not agree that an unreasonable risk to motor vehicle safety was present in their vehicles as a result of these recalled Caterpillar engines.These C7 engines were installed into 356 Chevrolet and GMC C-Series Medium Duty Trucks.GM stated that their underhood thermal safety team reviewed the situation on July 1, 2010 and concluded that a leak from the suspect pipe does not constitute a safety risk since no hot surfaces or ignition source is located near the potential leak.GM stated that it only had one warranty claim for repair of a C7 engine fuel line with no comments as to the root cause and no reference to any safety concern.GM also included a Powerpoint presentation from the company's safety analysis, which is included in the EQ repository.However, GM did provide its VIN list of affected vehicles to Caterpillar so Caterpillar could proceed with their recall remedy.GM's response will be forwarded to the Medium & Heavy Duty Vehicles Division for any further analysisThis EQ is closed.

(1) On November 3, 2010, DRV reported the complete separation of the left (driver side) - forward wheel assembly mounted to a 2010 model year DRV Mobile Suites RV equipped with aluminum wheels mounted on eight 9/16 wheel mounting studs.(2) On May 2, 2011, DRV reported the complete separation of the right (passenger side) - rear wheel assembly mounted to a 2010 model year Elite Suites RV equipped with aluminum wheels mounted on eight 5/8 wheel mounting studs.ODI, in conjunction DRV representatives, visited and inspected both of these vehicles shortly after receiving notification of the incident (before repairs were attempted) for the purpose of identifyingcontributory issues associated with components, component interfaces, and/or maintenance issues prior to pertinent evidence being lost or altered in the repair process.***** To improve the mounting surface quality in new production vehicles, DRV instructed that paint be removed from hub faces as of January, 2011.DRV also issued an owner letter in January, 2011 advising owners to check lug nut torque and notify DRV in the event of any owner concerns or problems regarding loosening or inability of the wheel retention system to maintain the specified torque.***** The occurrence of a wheel end separation incidents prompts concern about the wheel end retention integrity of DRV units in the field and justifies further investigation activity.

ODI has received 339 complaints alleging an unexpected loss of power steering assist while driving the vehicle.When the power steering assist stops working, drivers experienced difficulty in steering and/or controlling the vehicle.According to BMW drivers may experience either a temporary or permanent loss of power steering assist.BMW further advised that a temporary loss of power steering assist may be due to a malfunctioning cooling fan or an under-voltage condition in the power steering system.A permanent loss of power steering assist may be due to an error in the internal electronics, and may also result in the continuous running of the power steering pump.The VOQ complaints allege 5 minor crashes and 3 fires.The crashes are described as hit the curb.The fires are described as melted wires. There were no property damage claims.ODI has received similar complaints for other model year Mini.Additional information is required to evaluate the frequency, consequence, and scope of this issue.Accordingly the investigation has been upgraded to an Engineering Analysis (see EA11-005).
